SARS患者合并肺炎支原体感染的临床研究

摘　　要：目的探讨在本院由SARS(severeacuterespiratorysyndrome)冠状病毒引起的99例SARS患者合并肺炎支原体(Mycoplasmapneumoniae,MP)感染的临床情况变化。方法运用肺炎支原体抗体凝集实验,检测受SARS病毒感染的99例患者发病后的肺炎支原体抗体和滴度。结果99例SARS患者部分出现MP合并感染,阳性患者数为47例,总的阳性率为47.5%。在SARS发病前阶段,合并MP感染阳性率为69.3%(39/56),占整个阳性率的83.0%,并以高滴度分布较多;发病后阶段,合并MP感染阳性率为18.6%(8/43),占整个阳性率的17.0%,滴度较第一阶段偏低,两者有显著性差异(P<0.01)。结论SARS患者合并肺炎支原体感染,其感染率有阶段性差异,表明在前期感染SARS冠状病毒的患者毒性较强,对患者肌体影响较大,易出现肺炎支原体合并感染,而在后期的SARS冠状病毒感染者中,可能因毒性减弱,合并肺炎支原体感染率降低。Objective To study the significance of clinical changes of the infective SARS virus caused SARS combined with the infection of Mycoplasma pneumoniae(MP) in the Sun Yat sen memorial hospital. Methods Use the particle agglutination test for detection of anti Mycoplasma pneumoniae antibody (MP Ab) of 99 Atypical Pneumonia patients sera. Results Overall 47.5% SARS patients were also infected with Mycoplasma pneumoniae. At the early stage of SARS, 39 out of 56 SARS patients were MP positive (69.3%). It was 83.0% of the overall MP positive cases and with a high titer of antibodies. At the later stage, 8 out of 43 SARS patients were MP positive(18.6%). It was 17.0% of the overall MP positive cases. The anti MP antibody titer was lower than that at the early stage of infection(P< 0.01). Conclusion The infection rate of the combined infection with Mycoplasma pneumoniae in the SARS patients varied at different stages of the SARS. Data indicated that at the early stage of SARS, SARS virus might be more virulent than at the later stage. This could be related to the higher infection rate of Mycoplasma pneumoniae at the early stage than that at the later stage of SARS.
