部队军人及其驻地人群结核菌分离株分子标记分析

摘　　要：目的探讨部队军人与驻地地方人群之间结核病的传播关系。方法应用结核分支杆菌DNA插入序列IS6110作为引物,扩增结核分支杆菌参考株H37RVDNA,以同位素32P-dCTP标记纯化的245bp扩增片段作探针,应用Southern免疫转印技术,与预先用限制性内切酶PvuⅡ消化的结核分支杆菌DNA杂交,得到限制性片段长度多态性图谱,结合患者的一般流行病学资料进行分析。结果146株从部队和地方分离到的结核分支杆菌分离株,拷贝数在5以上的有138株,将这部分菌株按IS6110指纹特征分型可分为14个类型,部队和驻地患者分离株均以3个型别为主。且部队和地方分离菌株在各型中的分布无统计学差异。而耐药菌株指纹特征在三个主要型别中的分布与敏感菌株有显著性差异。结论部队结核病与驻地地方结核病在传播中有密切的关系。提示部队结核病防治应与驻地地方结核病防治措施紧密结合。Objective To study the molecular epidemiologic characteristics of Mycobacterium tuberculosis between the troops and the local population. Methods Using IS6110(which is a insert sequence of M. tuberculosis DNA) as primer to PCR amplify the reference strains H37RV DNA. The 245bp PCR fragment was labelled with 32P dCTP as probe to hybridize with PvuII digested M. tuberculosis DNA in the Southern blot analysis. The restriction fragment length polymorphism patterns and the patients epidemiological data were analyzed. Results A sample of 146 strains from the troops and the local population was studied. 138 strains of which had more than 5 copies. Among these isolates the RFLP patterns could be divided into 14 types. There were three main type isolates identified among the army and local population, type Ⅰ (23.9%), type Ⅱ(18.1%), and type Ⅲ(21.7%). The other types were less than 8 percentage. The distribution of the types between the troops and the locals showed no statistically significant difference. But the distribution of the drug resistant fingerprint patterns and the drug sensitive fingerprint patterns in the troops main types showed a significant difference. Conclusions The transmission of tuberculosis in the troops was closely correlated with that of the locals. Prevention and cure of tuberculosis in the troops should combine with that in the locals.
