针刺康复为主导治疗亨特综合征临床观察

摘　　要：目的:观察针刺康复为主导的治疗方案治疗亨特综合征的临床疗效。方法:将发病3天以内的140例亨特综合征患者随机分为治疗组55例,对照组55例和药物组30例。治疗组:初期针刺以面部少阳经穴、面部肌肉被动康复为主配合中药龙胆泻肝汤加减和Harrison,s方案,中、后期采用单纯的针刺阳明经穴为主和主动康复治疗方案;对照组:初、中、后期均采用针刺面部阳明经穴、按摩面部肌肉的被动康复治疗,初期配合中药牵正散和《实用内科学(14版)》药物方案;药物组:初期采用治疗组中西药方案,全程配合口服营养神经药物和按摩面部肌肉的被动康复治疗;各组初期观察为10天,中期及后期各为20天,共观察50天;以面部肌肉恢复症状及House-Brackmann分级评分标准为观察指标。结果:3组在治愈率、总有效率、House-Brackmann分级评分标准,均有显著性差异(P<0.01)治疗组优于其他各组。结论:初期尽力保护神经轴索,消除骨管水肿,降低轴索损害,减小面肌失神经症状,中、后期加强面神经功能恢复的针刺康复为主导的治疗方法优于传统的治疗方法和单纯药物疗法。Objective: To analyze clinic treatment on Hunt syndrome by acupuncture rehabilitation. Methods: 140 cases suffered from Hunt syndrome within 3 days course were randomly distributed into three groups: treatment group with 55 cases,controlled group with 55 cases and medical group with 30 cases. The treatment group: acupuncture face Shaoyang point at initial stage,face muscle passive recovery cooperated by Longdan Xiegan Decoction plus and minus and Harrison's plan. Acupuncture Yangming meridian and points and active recovery in the mid and late stage. The controlled group: acupuncture facial Yangming meridian and points,massage facial muscle recovery throughout the course,cooperated by Qianzheng Powder and medical treatment by the utility of department of internal medicine( the 14 th edition). The medical group: using treatment group of method at first,cooperated by oral administration of neurotrophic drugs and massage facial muscle recovery throughout the course. 10 days observation for initial stage,20 days for the mid and late stage,total for 50 days. The observation index was based on face muscle recovery and House-Brackmann's score. Results: Recovery rate: the total efficiency and House-Brackmann's score for treatment group was better than other groups. There were very significant differences( P < 0. 01). Conclusion: Trying our best to protect axonal,reduced axonal damage,the reduction of facet muscle denervation symptoms in initial stage,the treatment of acupuncture recovery to strengthen the function of the facial nerve dominated in the mid and late stage was better than that of traditional method and pure medical therapy.
