腰椎间盘突出症红外热图特征及粗银针治疗临床研究

摘　　要：目的:观察粗银针治疗腰椎间盘突出症(lumbar disc herniation,简称LDH)的临床疗效。探讨医用红外热成像(infrared thermography,简称IRT)在LDH的诊断和疗效评定中的价值。方法:选取30例LDH患者进行IRT检查,观察腰骶及双下肢双侧温差情况,并进行比较分析。对30例LDH患者采用粗银针针刺腰骶部压痛点的方法进行治疗。治疗1个疗程后,进行自身前后对照,观察粗银针治疗LDH的临床疗效。再行IRT检查,进行自身前后对照,分析腰椎间盘突出症患者红外热像图特征。结果:粗银针对LDH有着显著的疗效。健康组与疾病组IRT比较发现,腰骶部及下肢红外热像图双侧温差在特定区域有显著性差异。治疗组在治疗前后腰骶部及下肢红外热图双侧温差缩小。结论:粗银针治疗LDH是一种安全、有效的非手术疗法。IRT能直观地反映正常人体表面的温度分布及LDH患者异常热区的分布,可作为LDH诊断及疗效评定的功能性指标。Objective: To observe clinical curative effect of thick silver needle treatment for lumbar disc herniation( LDH)and had a discussion on the value of applying infrared thermography( IRT) for LDH diagnosis and curative effect assessment.Methods: A comparative analysis was done on the IRT image of 30 cases of normal people and 30 cases of LDH patients; observing the bilateral temperature difference in regions of lumbar-sacrum and lower limbs of the two groups by IRT. Applying thick silver needle treatment on tender points in lumbar-sacrum for 30 cases of LDH patients for a period of treatment. And had a observation on the curative of thick silver needle. As well as an IRT check again to analyze the features of IRT of LDH patients. Results: Thick silver needle treatment was evident in curative effect for LDH. By the IRT comparison of the controlled group and the treatment group,the bilateral temperature difference in regions of lumbar-sacrum and lower limbs of the two groups had obvious difference. In addition,the temperature difference of the treatment group in regions of lumbar-sacrum and lower limbs of the two groups was decreased. Conclusion: Thick silver needle treatment is a safe,practical and effective non-surgical therapy for LDH. IRT is accurate and instinct in reflecting temperature distribution on body surface of normal people and LDH patients,which is acceptable as a functional index for LDH diagnosis and assessment of curative effect.
