自拟土丹汤治疗急性痛风性关节炎及高尿酸血症疗效观察

摘　　要：目的:观察自拟土丹汤治疗急性痛风性关节炎及高尿酸血症的临床效果。方法:采用随机数字表法将我们医院2012年10月~2014年10月收治的117例痛风性关节炎合并高尿酸血症患者分为中医组58例和对照组59例,两组患者均予以西医常规药物治疗,中医组加用自拟土丹汤治疗,疗程21天,对比两组的治疗效果。结果:治疗前中医组和西医组关节疼痛、关节肿胀、关节压痛、关节活动障碍症状评分、各项中医证候积分、ESR、CRP、WBC、SUA测定值差异无统计学意义(P>0.05);治疗后两组关节疼痛、关节肿胀、关节压痛、关节活动障碍评分、各项中医证候积分、ESR、CRP、WBC、SUA测定值较本组治疗前均显著的降低(P<0.05),治疗后中医组关节疼痛、关节肿胀、关节压痛、关节活动障碍评分、各项中医证候积分、ESR、CRP、WBC、SUA测定值均显著低于西医组(P<0.05);中医组控显率(77.59%)显著高于西医组(57.63%)(P<0.05),中医组总有效率(94.83%)高于西医组(89.83%),但差异无统计学意义(P>0.05)。结论:自拟土丹汤结合西医常规治疗急性痛风性关节炎及高尿酸血症的临床效果优于单纯西医疗法。Objective: Observing the effect of using Self-made Tudan Decoction in the treatment of acute gouty arthritis and hyperuricemia. Methods: 117 patients with gouty arthritis hyperuricemia practitioners gathered from October 2012 to October 2014 of our hospital were divided into 2groups,58 cases in the TCM group and 59 cases in the controlled group with random number table method,two groups of patients were both given western conventional drug therapy,the TCM group was given Self-made Tudan Decoction,the treatment course was 21 days,the therapeutic effect of the two groups was compared. Results: Before treatment,the traditional Chinese medicine and the western medicine group patients of joint pain,joint swelling,joint tenderness,joint movement disorder symptom score,the symptom score,ESR,CRP,WBC,SUA measured value was no significant difference( P>0. 05); after treatment,the patient’s joint pain,joint swelling,joint tenderness,joint movement disorder scores were significantly reduced the symptom scores,ESR,CRP,WBC,SUA measured value compared with this group before treatment( P <0. 05),the Chinese medicine group after treatment of patients with joint pain,joint swelling,joint tenderness,joint movement disorder score,the symptom score,ESR,CRP,WBC,SUA measured values were significantly lower than the western medicine group( P <0. 05); Chinese medicine control group of patients was 77. 59 percent rate was significantly higher than 57. 63%( P <0. 05),the total efficiency of the board 89. 83%,94. 83% higher than the western medicine group,but the difference was not statistically significant( P >0. 05). Conclusion: Using Self-made Tudan Decoction combined with conventional western medicine in the treatment of acute gouty arthritis and hyperuricemia has better effect than using western medicine method only.
