胃癌根治术中瑞芬太尼复合丙泊酚应用右美托咪定对全麻药量及麻醉效果的影响

摘　　要：目的探讨胃癌根治术中瑞芬太尼复合丙泊酚应用右美托咪定对全麻药量及麻醉效果的影响。方法将60例行全麻胃癌根治术患者随机分为实验组和对照组。对照组采用常规全麻麻醉,实验组患者在此基础上加用右美托咪定进行麻醉,比较2组患者不同时刻BIS、MAP、HR、VAS评分和Ramsay评分变化情况,以及麻醉药物剂量和不良反应发生情况。结果实验组在T1、T3~T6时HR和MAP均低于对照组,差异有统计学意义(P<0.05)。但实验组仅在T1时BIS低于对照组,差异有统计学意义(P<0.05)。实验组患者术后1 h和术后4 h时VAS评分均低于对照组患者,而Ramsay评分高于对照组患者,差异均有统计学意义(P<0.05)。实验组患者丙泊酚和瑞芬太尼用量均显著低于对照组,差异有统计学意义(P<0.05)。治疗过程中2组患者均未发现不良反应的发生。结论右美托咪定可减少麻药剂量,提高全麻麻醉的镇痛、镇静效果,安全性良好,可作为临床常规应用。Objective To evaluate influence of dexmedetomidine in general anesthesia underwent gastrectomy with remifentanil and propofol on dosage and anesthesia effect. Methods 60 cases of patients underwent gastrectomy were divided into the experimental group and the control group. The control group were treated with routine general anesthesia,the experimental group were treated with dexmedetomidine on the basis of the control group. The BIS,MAP,HR,VAS and Ramsay score,anesthesia dosage and adverse reactions between the 2 groups were compared. Results In the experimental group,HR and MAP at T1,T3~T6were lower than those of the control group,the difference was statistically significant( P < 0. 05). But in the experimental group BIS only was lower than that of the control group at T1,the difference was statistically significant( P < 0. 05). Patients in the experimental group 1 h and 4 h after operation,VAS scores were lower than those of the control group,and Ramsay score were higher than that of the control group,the difference was statistically significant( P < 0. 05). Patients in the experimental group,propofol and remifentanil dosage were significantly lower than those of the control group,the difference was statistically significant( P <0. 05). Patients of the 2 groups had no adverse reactions. Conclusion Dexmedetomidine can reduce anesthetic dose,improve anesthesia analgesic,sedative effect,it can be used as a routine clinical application.
