颈动脉CT血管成像在短暂性脑缺血发作患者中的应用

摘　　要：目的分析短暂性脑缺血发作(TIA)患者颈动脉CT血管成像特征,探讨颈动脉CT血管成像在TIA中的应用价值。方法回顾分析40例TIA患者颈动脉CT血管成像资料分析颈动脉斑块的分布、性质及形态特征测量颈动脉狭窄程度并与40例脑梗死患者进行对照比较。结果 40例TIA患者中存在颈动脉斑块90块,其中不稳定斑块31块,稳定斑块59块。40例患者中颈动脉出现轻度狭窄80处,中度狭窄8处,重度狭窄2处。TIA与脑梗死组颈动脉斑块构成的差异有统计学意义(X^2=4.39,P<0.05),TIA与脑梗死组颈动脉狭窄程度比较差异无统计学意义。结论颈动脉CT血管成像能够准确判断颈动脉斑块的分布、性质、形态特征及颈动脉的狭窄程度,对TIA的病因诊断、治疗及预后的判断具有重要价值。Objective To analyze 64-slice CT imaging features of carotid atherosclerotic plaque and explore relationship between different types of carotid atherosclerotic plaque and transient ischemic attack (TIA). Methods The clinical data of 40 cases of TIA were retrospectively analyzed, all the cases underwent 64-detector row CT an-giography ,and observe distribution, character and morphological characteristics of carotid atherosclerotic plaque, and morphological characteristics and measure the extent of stenosis with the original axial image, volume rendering (VR) and curved planar reconstruction (CPR), which were compared with 40 cases of cerebral infarction. Results Of 40 cases with TIA, 90 plaques were found, including 31 unstable plaques and 59 stable plaques. Of 40 cases, there were 80 mild stenosis, 8 moderate stenosis, 2 severe stenosis were found. There was significantly statistical difference in plaque types between TIA and cerebral infarct ( χ2=4.39, P<0.05). There was no significantly statistical difference in carotid stenosis between TIA and cerebral infarct. Conclusion 64-detector row CT angiography of carotid atherosclerotic plaques can accurately detect the distribution, character and morphological characteristics of carotid atherosclerotic plaque and the degree of stenosis of carotid artery, it is a valuable method in diagnosis, treatment and prognosis judgement of TIA.
