肠粘连松解汤联合西药治疗粘连性肠梗阻随机平行对照研究  

Changnianliansongjie Decoction Combined with Western Medicine in the Treatment of Adhesive Intestinal Obstruction Randomized Parallel Controlled Study

摘　　要：[目的]观察肠粘连松解汤联合西药治疗粘连性肠梗阻疗效。[方法]使用随机平行对照方法,将76例门诊患者按就诊顺序编号法随机分为两组。对照组38例常规治疗,禁食,进行胃肠减压,补液纠正水、电解质以及酸碱平衡,抗生素,解痉、止痛,防止腹腔感染。治疗组38例肠粘连松解汤(生大黄10g,厚朴15g,当归、木香、桃仁、青皮、莱菔子炒、枳壳、乌药12g,甘草15g);梗阻时间较长,怀疑有肠道细菌移位的,加金银花、蒲公英各18g,1剂/d,水煎200mL,早晚口服。西药治疗同对照组。连续治疗7d为1疗程。观测临床症状、血生化、血常规、尿常规、不良反应。连续治疗3疗程,判定疗效。[结果]治疗组痊愈21例,显效7例,有效8例,无效2例,总有效率94.70%。对照组痊愈17例,显效6例,有效10例,无效5例,总有效率86.80%。治疗组疗效优于对照组(P<0.05)。[结论]采用肠粘连松解汤联合西药治疗粘连性肠梗阻疗效显著,值得临床推广应用。[Objective]Observed Changnianliansongjie decoction combined with western medicine treatment of adhesive intestinal obstruction effect.[Methods]Using randomized controlled method,76 cases of outpatient treatment order numbers were randomly divided into two groups.Control group,38 cases of conventional therapy,fasting,for decompression,rehydration correcting water,electrolyte and acid-base balance,and given antibiotics,according to the condition appropriate to give antispasmodic and painkillers,to prevent abdominal infection.Treatment group 38 cases take blood heat,purgative,qi pain TCM treatment.Changnianliansongjie decoction(Shengdahuang 10g,Houbu 15g,Danggui,Muxiang,Taoren,Qingpi,Laifuzi fried,Zhike,Wuyao 12g,Gancao 15g).Obstruction longer suspected bacterial translocation,plus honeysuckle,dandelion 18g.1 does / d,decoction 200mL,sooner or later.Western medicine with the control group.7d is a continuous course of treatment.Observation of clinical symptoms,blood chemistry,blood,urine,adverse reactions.Continuous treatment of 3 courses,to determine efficacy.[Results]The cure 21 cases,7 cases were markedly effective,8 cases,2 cases,the total efficiency of 94.70%.Control group cured 17 cases,effective in 6 cases,10 cases,5 cases ineffective,the total effective rate was 86.80%.Treatment group than the control group(P <0.05)[.Conclusion]Using Changnianliansongjie decoction combined with western medicine treatment of adhesive intestinal obstruction effect is significant,worthy of clinical application.
