摘 要:目的 :探讨血清、诱导痰嗜酸粒细胞阳离子蛋白 (ECP)与哮喘气道炎症及哮喘严重程度的关系。方法 :40例哮喘患者 (轻度 1 6例 ,中、重度 2 4例 )经普米克治疗前后分别行血清、诱导痰ECP以及肺功能第一秒用力呼气容量 (FEV1 )检测 ,并同时与正常人群血清、诱导痰ECP比较。结果 :哮喘患者治疗后血清、诱导痰ECP明显下降 (P <0 .0 1 ) ,肺功能FEV1 明显上升 (P <0 .0 1 ) ;哮喘患者治疗前 (发作期)血清、诱导痰ECP均明显高于正常人群 (P <0 .0 1 ) ,治疗后 (缓解期 )诱导痰ECP仍明显高于正常人群 (P <0 .0 1 ) ,血清ECP则稍高于正常人群 (P >0 .0 5 ) ;中、重度哮喘血清、诱导痰ECP明显高于轻度哮喘 (P <0 .0 5 ) ;血清、诱导痰ECP与FEV1 呈显著负相关 (r=-0 .48、-0 .5 0 ,P <0 .0 5 )。结论 :血清、诱导痰ECP可反映哮喘气道炎症以及严重程度 ,可用于哮喘气道炎症的研究以及指导治疗 ,诱导痰ECP比血清ECP更敏感。AIM: To explore the relationship among serum and induced sputum eosinophil cationic protein (ECP), asthmatic airway inflammation and the severity of asthma. METHODS: 40 patients with asthma (16 patients with mild asthma, 24 patients with moderate to severe asthma) were observed. Serum and induced sputum ECP and the forced expiratory volume in one second (FEV 1) were measured before and after treatment with pulmicort, which were compared with those in normal subjects. RESULTS: In patients with asthma after the treatment serum and induced sputum ECP levels decreased significantly (P< 0.01 ) and FEV 1 increased significantly (P< 0.01 ). Before the treatment (exacerbation stage) serum and induced sputum ECP levels in patients with asthma were significantly higher than those in normal subjects (P< 0.01 ), however serum ECP levels were slightly higher than those in normal subjects (P> 0.05 ). Serum and induced sputum ECP levels in patients with moderate to severe asthma were significantly higher than those in patients with mild asthma (P< 0.05 ); the levels of serum and induced sputum ECP were negatively correlated with FEV 1 (r=- 0.48 and - 0.50 , P< 0.05 ). CONCLUSION: The levels of serum and induced sputum ECP may reflect the severity of asthma and asthmatic airway inflammation, and these can be used in the investigation of asthmatic airway inflammation and guiding therapy. Induced sputum ECP is more sensitive than serum ECP.
