骨科下肢侧卧位手术牵引架的研制及临床应用

摘　　要：目的 :研制并临床验证一种简便、实用、灵活、科学的骨科下肢侧卧位手术牵引架。方法 :1 999年 1月～ 2 0 0 3年 1月采用本手术设备共进行了 5 1例均在侧卧位状态下进行的手术 ;其中股骨粗隆间骨折 2 3例、股骨粗隆下骨折 4例 ,行 DHS2 2例、Gamma钉 5例 ;股骨中下段骨折 2 4例 ,行股骨交锁钉 3例、梅花钉 3例、LC-DCP1 8例。结果 :采用本手术设备的手术情况是 :DHS、Gamma钉组的平均手术时间为 5 1± 1 9min、出血量为 1 0 0± 5 7ml,交锁钉组的手术时间为 70± 1 9min、出血量为 3 1 2± 94ml;与常规的仰卧位手术比较 P<0 .0 1 ,同时方便 C-臂 X光机透视 ,尤其是侧位透视。此外 ,还可运用本手术设备进行股骨中下段骨折的其他各种内固定手术 ,牵引效能好 ,方便术区皮肤消毒及术中骨折复位。结论 :本骨科下肢侧卧位手术牵引架结构简单、零部件少、装卸容易、适应症广、操作方便、利于 C-臂 X光机透视 ,术中出血量少、损伤小、手术时间短 ,具有较好的实用型、先进性、科学性 ,值得临床推广应用。Objective:Developing and Clinical applying a kind of easy, practical, elastic, scientific operation tractor frame for orthopaedics with lateral posture.Methods:From January 1999 to January 2003, 51 cases were operated using this equipment, including 23 intertrochanteric fractures, 4 infratrochanteric fractures (treating with DHS 22cases,Gamma nail 5 cases )and 24 fractures of femurs shaft in king-inferior segment treating with interlocking medullary nail 3 cases, plum blossom nail 3 cases, LC-DCP 22 cases.Results:Mean operation time was 51±19min and volumes of bleeding were 100±57ml in group DHS and Gamma nail by this equipment. But 70±19min and 312±94ml in group interlocking medullray nail. The operation time was shorter and volumes of bleeding were less than those of supine posture (P<0.01). AT the same time ,it was convenient to fluoroscopy by C-arm X ray machine. Furthermore, this equipment can be used the interfixation operations of femur shaft fracture with good traction effect, easy to sterilize the skin and to reduce fracture.Conclusion:the equipment has not only with simple structure, easy installment and dismantle, widely indication, but also other merits, such as less bleed volume and harm, short operation time, convenient fluoroscopy by using the equipment. So it was practical advantageous scientific and worth to widely use in clinic.
