乳腺肿块细针穿吸细胞学及AgNOR图像分析技术的价值

摘  要:目的 讨论FNAC结合AgNOR真彩色图像分析对乳腺肿块的诊断价值。方法 总结86例有手术后或活检病理诊断对照的乳腺肿块FNAC检查结果,全部病例行AgNOR真彩色图像分析。结果 FNAC查见癌细胞者27例,可疑恶性病变4例,良性病变49例,非典型病变6例,假阴性2例,良恶性诊断正确率97.67%。良恶性病变相比,AgNOR颗粒平均个数>3个者,比例、直径≥2.5μm者,比例、平均面积、平均积分光密度等均有显著性差异。结论 FNAC是确诊乳腺肿块性质的有效方法。如果结合AgNOR图像分析技术,将具有更广阔的前景。Objective To evaluate fine needle aspiration cytology (FNAC) in the diagnosis of breast masses. Methods A comparison between pathological diagnosis and cytological diagnosis in 86 cases of breast masses was studied. All cases were studied with AgNOR image analysis. Results In 86 cases by FNAC, there were 27 carcinoma, 4 equivocal malignant tumors, 49 benign pathological changes, 6 dis-typical pathological changes, 2 false negative. The rate of definite diagnosis was 97.67% . In the comparisons between benign and malignant pathological changes, the mean area, integral optical density, proportions of the cells with AgNOR granular diameter ≥2.5μm and mean number of granules > 3 from benign cells were significantly different from those of malignant cells. Conclusion FNAC is of great value in the diagnosis of breast masses. The AgNOR quantitative analysis may be used as a new technique of cytology.
