盐酸关附甲素注射液对室性早搏的疗效与安全性观察

摘　　要：目的 :观察盐酸关附甲素对室性早搏 (室早 )的疗效与安全性。方法 :入选 2 0 0 1年 2月至2 0 0 2年 8月在我院心内科住院的室性早搏患者 3 5例 ,按 4∶1的比例 ,随机分为关附甲素组与普罗帕酮组 ,按个体化原则给予负荷量静脉推注后 ,给予持续静脉维持。结果 :患者平均年龄 (5 2 8± 12 5 )岁 ,其中男性 13例 (3 7 1% ) ,女性 2 2例 (62 9% ) ,随机分关附甲素组 2 8例 ,普罗帕酮组 7例。关附甲素的总有效率为 85 7% ,普罗帕酮为 5 7 1%。 2组患者用药前、中、后的血压无明显变化。关附甲素可明显减慢患者心率 ,由用药前的 (79 2± 12 3 )次 min减少至 (69 0± 8 7)次 min(P =0 0 0 2 ) ,这种减慢心率的作用可于停药后 2 4h恢复正常。关附甲素组中 10例出现不良反应 ,其中最常见的为舌麻。关附甲素及普罗帕酮 2种药物均有使PR间期及QRS时相延长的作用 ,但这种作用在前者更为显著 ,关附甲素对患者的QTc无明显影响 ,而普罗帕酮延长QTc。结论Objective:To observe the efficacy and safty of Guan-fu base A in treatment of patients with ventricular premature beats.Method:35 patients with ventricular premature beats who were addmitted in our hospital from Feb 2001-Aug 2002 were randomized to treament of Guan-fu base A or propafenone (the ratio was 4∶1). After a bolus injection, all patients received maintenace infusion according to the patients' body weight.Result:The mean age of the patients was 52.8±12.5, 13 was male(37.1%). 28 patients were treated with Guan-fu base A and 7 patients with propafenone. The overall effective rate of Guan-fu base A was 85.7% and that of propafenone was 57.1%. This two drugs had no on blood pressure. Guan-fu base A could make the patients' heart rate lower (69.0±8.7 vs 79.2±12.3, P=0.002), but after the treatment stopped, this effection could recovered. 10 adverse effect occurred in the treatment of Guan-fu base A, the most common one was tongue numbness. Both drugs could prolonged PR interval and QRS duration, especially Guan-fu base A. Guan-fu base A has no effection on QTc interval but propafenone could prolonged it. Conclusion:In treatmeat of ventricular premature beats, Guan-fu base A was safe and effective.
