瘦素抵抗与老年男性肥胖高血压的关系

摘　　要：目的 :　研究老年男性肥胖高血压患者血清瘦素和胰岛素水平的变化 ,探讨瘦素抵抗与老年男性肥胖高血压的关系。方法 :　用放射免疫法测定 41例高血压和 2 1例正常血压的老年男性患者血清瘦素和胰岛素的含量。结果 :　在高血压和肥胖分组中空腹血糖变化无显著性差异。非肥胖组高血压与正常血压者血清瘦素和胰岛素的水平差别无显著性。肥胖组高血压者较正常血压者血清瘦素和胰岛素含量分别平均升高 1 .8μg/L和 2 .7m IU/L(P<0 .0 1 )。高血压组中肥胖者较非肥胖者血清瘦素和胰岛素水平分别平均升高 2 .7μg/L和 4.7m IU/L (P<0 .0 0 1 )。 70岁以上者较 6 0～ 70岁者血清瘦素水平平均升高 1 .7μg/L(P<0 .0 5 )。结论 :　老年肥胖高血压存在瘦素抵抗和胰岛素抵抗 ,瘦素与伴有肥胖的高血压有密切关系。Objective: To study the content of serum leptin and insulin in elderly men with obesity-related hypertension, and analyse the relationship between leptin resistance and obesity-related hypertension.Methods: The concentrations of serum leptin and insulin in 41 hypertensive cases and 21 normotensive cases of senile men were determined by radioimmunoassay.Results: The changes of fasting blood glucose in hypertensive groups or obese groups were not significant. The contents of serum leptin and insulin between hypertensives and normotensives in non-obese group were not significantly different. Compared with that of normotensives, the concentrations of serum leptin and insulin in obesity group remarkablely increased 1.8 μg/L and 2.7 mIU/L respectively(P<0.01).The levels of serum leptin and insulin in obesity-related hypertension were markedly higher than those in non-obesity-related hypertension, and elevated 2.7 μg/L, 4.7 mIU/L respectively. The content of serum leptin in hypertensives with age >70 years old was 7.8±2.2 μg/L, whereas 6.1±2.7 μg/L in age≤70 years old (P<0.05).Conclusion: Leptin-resistance and insulin-resistance exist in obesity-related hypertension. The relationship between leptin and obesity-related hypertension is very close.
