内服与直肠滴注中药配合导管扩通术治疗输卵管阻塞性不孕症的临床观察

摘　　要：[目的]观察内服与直肠滴注中药配合导管扩通术治疗输卵管阻塞性不孕症的效果及对术后再粘连的作用。[方法]52例输卵管阻塞性不孕症患者随机分为中药组和西药组。两组均于X线下行输卵管导管扩通术治疗。中药组配合口服通管助孕汤(主要由柴胡、枳实、赤芍、丹参、三七、穿山甲、路路通、麦冬、甘草等组成)与直肠滴注直肠滴注方(主要由丹参、赤芍、三棱、乳香、没药、枳实等组成);西药组于术后第2～3天以地塞米松针剂5 mg、玻璃酸酶1 500U,加0.15 mmoL/LNaCl注射剂20mL进行宫腔注射,比较两组扩通术后输卵管再粘连率、妊娠率的差异性。[结果]中药组与西药组导管扩通有效率分别为96.5％与95.1％(P>0.05);输卵管再粘连率分别为10.7％和17.9％(P<0.01),妊娠率分别为53.3％和27.2％(P<0.01)。[讨论]中药内服与直肠滴注可比西药更有效减少输卵管导管扩通术后再粘连,提高妊娠率。[ Objective ] To observe the therapeutic effect of Chinese herbal medicine for oral administration and intrarectal drip combined with transvaginal catheter dilation (TCD) for tubal obstruction infertility (TOI) and to investigate their actions on tube re-adhesion after TCD. [Methods] Fifty-two cases of TOI were randomized into Group A ( n = 30) and Group B ( n = 22). TCD was carried out under X-ray in both of the two groups. Additionally, Group A was treated with Tongguan Zhuyun Decoction orally and intrarectal drip of Zhichang Dizhu prescription and Group B treated by intrauterine injection of dexamethasone 5 mg, hyaluronidase 1 500 U and normal saline injection 20 mL 2-3 days after TCD. Tube re-adhesion and pregnancy rate were compared in the two groups after TCD. [Results] Effective rate of TCD was 96.5% and 95.1% in Group A and Group B respectively ( P > 0.05) . Tube re-adhesion rate was 10.7% and 17.9% (P<0.01) and pregnancy rate 53.3% and 27.2% (P<0.01) respectively in Group A and Group B. [Conclusion] TCD is effective for TOI and the combination of Chinese herbal medicine for oral administration and for intrarectal drip can relieve the tube re-adhesion and increase the pregnancy rate.
