Analysis on the Alteration of Ventricular Wall Motion Velocity by Doppler Tissue Imaging in Patients With Coronary Heart Diseases

摘  要:目的:采用多普勒成像技术(DTI)对冠心病患者及健康人进行局部室壁运动速度测定,以了解DTI是否可作为反映局部功能的检查手段。方法:本文选择了经冠状动脉造影证实的冠心病患者36例作为研究组,另取20例健康人作为对照组,分别测定DTI收缩期峰值速度(Vs),舒张早期峰值速度(Ve),舒张晚期峰值速度(Va)及Ve/Va等指标,将各病变室壁指标与对照组相应指标比较,以了解DTI是否可反映局部室壁的收缩、舒张功能变化。结果:前壁病变组Vs、Ve、Va、Ve/Va均低于对照组(P<0.05);下壁病变组Vs、Ve、Ve/Va也显著低于对照组(P<0.01或P<0.05);而侧壁病变组的Vs、Va与对照组比较无统计学差异,Va、Ve/Va较对照组明显降低(P<0.05)。结论:DTI能很准确地将心脏室壁运动的信息实时显示出来,冠脉组病变室壁的Vs、Ve/Va明显低于对照组,因而Vs可反映局部收缩功能的变化,Ve、Ve/Va可反映局部舒张功能的变化。Objective: Doppler Tissue Imaging (DTI) was used to measure the local wall velocity of CHD patients and normal individuals, and to testify if DTI can reflect systolic and diastolic function change of location wall. Methods: Thirty-six CHD patients identified by coronary angioplasty were enrolled in this study as CHD group and 20 normal subjects served as control group. The systolic velocity (VS), early diastolic peak velocity (Ve), late diastolic peak velocity (Va), Ve/Va were respectively measured and compared between CHD group and control group. Results: Vs, Ve, Va, Ve/Va in dysfunctional inferior wall patients were respectively lower than that in control group (P< 0.05 or P< 0.01) ; Ve, Ve/Va in dysfunctioal lateral wall patients were significantly lower than that in control group (P< 0.05), while Vs, Va showed no difference. Conclusion: DTI can actually display in real time the information of cardiac motion, Vs, Ve, Ve/ Va in CHD group were lower than control group, Vs can reflect local systolic function alterations, while Ve, Ve/Va can reflect local diastolic function changes.
