心脉通胶囊对急性心肌梗塞患者存活心肌保护作用的临床观察

摘　　要：目的:研究中药复方心脉通胶囊对急性心肌梗塞(AMI)患者缺血存活心肌的保护作用。方法:将成功进行PTCA术的AMI患者16例随机分为治疗组(23例)和对照组(23例),在AMI常规治疗基础上,治疗组在术前3天开始服用心脉通胶囊,治疗1周,观察治疗前后核素静息心肌显像节段数、核素心肌灌注缺损范围分数(ES)和核素心肌灌注缺损严重程度积分(SS)以及循环血血管内皮生长因子(VEGF)水平的变化。结果:记分为1的静息心肌显像节段数转变2分的变化率治疗组(58.33％)明显大于对照组(53.33％)(P<0.05),ES、SS治疗组明显小于对照组(P<0.05):术后3天和1周VEGF浓度治疗组明显高于对照组(P<0.05)。结论:中药复方心脉通胶囊能促进存活心肌的代谢活动,促进循环VEGF的表达,对AMI患者缺血存活心肌起积极保护作用;益气活血中药有血管新生活性样作用,有助于梗塞区新生血管的形成和侧支循环的建立,对改善缺血存活心肌的低灌注状态将起重要作用。Objective: To study protective action of TCM complex prescription Xinmaitong Capsules on survival myocardium in the patient of acute myocardiac infarction (AMI). Methods: 46 cases of AMI who had successfully received PTCA operation were randomly divided into treatment group and control group with 20 cases in each. On the basis of routine treatment of AMI. 3 days before the operation, treatment group began to take Xinmaitong Capsules for I weeks. Nuclide resting myocardium development segment numbers, nuclide myocardium perfusion defect range score (ES) and nuclide myocardium perfusion defect severe extent cummulative scores (SS). as well as the level of blood vascular endothelial growth factor (VEGF) were investigated before and after treatment. Results: The varying rate of the resting myocardium development segment number from score 1 to 2 was 58. 33% in treatment group, significantly higher than 53. 33% in control group (P<0. 05). and ES and SS in treatment group was significantly lower than those in control group (P<0. 05). VEGF levels of 3 days and one week after the operation in treatment group were significantly higher than those in control group (P<0. 05). Conclusion: Xinmaitong Capsules can promote metabolic activity of the survival myocardium and expression of blood VEGF, exerting positive protective action on ischemic myocardium in the patient of AMI. Chinese drug for supplementing Qi and promoting blood circulation is of action like blood vessel neogenesis activity . contributing to formation of neonatal vessels and establishment of collateral circulation in the infarction region.which plays an important role in improvement of low perfusion state in ischemic survival myocardium.
