摘  要:目的 :探讨低潮气量、呼吸末正压 (PEEP)机械通气在急性呼吸窘迫综合征 (ARDS)的应用。方法 :选择 2 4例ARDS患者进行回顾性病例分析 ,比较传统潮气量机械通气与低潮气量十PEEP机械通气治疗的两组病例血气指标、中心静脉压 (CVP)、血氧饱和度 (SpO2 )及出现多器管功能不全(MODS)的时间。结果 :两组中血气指标、CVP、SpO2 ,无显著性意义。出现MODS时间氏潮气量十PEEP机械通气组明显延长 (P <0 .0 5 )。结论 :低潮气量 ;PEEP机械通气在ARDS患者治疗中 ,改善氧合状况与传统方法无差异 ,但因能比较有效地防止机械通气相关肺损伤 。Objective: To investigate the efficacy of mechanical ventilation with low tidal volume and positive expiratory end pressure(PEEP)in the treatments of acute respiratory distress syndrome(ARDS). Methods: 24 patients with ARDS were divided into two groups. Mechanical ventilations of low tidal volume and PEEP were used in the study group while traditional mechanical ventilations were applied in the comtrol group. The index of blood gas analysis, central venous pressure(CVP), degree of blood oxygensaturation(SpO 2) and latent period of nlultiple organ dysfunction syndronme(MODS) were observed. Results: There is no statistical difference in the index of blood gas analysis.CVp and SpO 2 between the two groups, but the latent periods of MODS in the study groups were longer than those of the control group. Conclusion: Mechanical ventilationwith low tidal volume and PEEP can reach the same efficacy of oxygen saturation as mechanical ventilations in ARDS, further more, it can prevent the ventilator-induced lung injury, so it can be applied in the treatment of ARDS.
