单纯性肾病综合征血T细胞亚群变化及对体液免疫功能的影响

摘　　要：对39例初发和复发单纯性肾病综合征病儿外周血T细胞亚群,IgG,IgA,IgM进行检测.结果表明,无论初发或复发病例外周血中CD_3^+和CD_4^+细胞水平无明显变化,CD_8^+和CD_(10)^+细胞均明显高于对照组,CD_4^+/CD_8^+细胞比值均明显低于对照组,而初发组和复发组间无明显差异;85.3%的病儿伴血清IgG降低,29.4%的病儿伴血清IgM增高,CD_4^+/CD_8^+细胞比值与血清IgG水平呈显著正相关,而与IgM水平呈明显负相关.提示单纯性肾病综合征病儿存在T细胞功能紊乱,且与病儿体液免疫功能紊乱的关系密切.The T cell subsets of peripheral blood and serum level of immunoglobulins in 39 cases of simple nephrotic syndrome (NS) were determined. The results were as follows : In simple NS the numbers of CD_3^+and CD_4^+cells of peripheral blood remained unchanged while those of CD_8^+ and CD_(10)^+ cells were found significantly greater than in the control group. The ratio of CD_4^+/CD_8^+ cells was significantly lower in simple NS and in the control group. But there was no significant difference either in the levels of CD_8^+ and CD_(10)^+ cells or in the ratio of CD_4^+/CD_8^+ cells between the incipient group and the recurrent group. The serum IgG in 85.3% of the cases was decreased and IgM in 29.4% was increased. The ratio of CD_4^+/CD_8^+ cells had a significant positive correlation with the level of serum IgG but a negative correlation with the level of serum IgM. The results suggest that the T cell functions in simple NS are disordered and this is closely related to humoral immune disorder.
