北京市210例脑血管疾患老人的家庭护理需求

摘　　要：目的 :对北京市脑血管疾患老人 (6 0岁以上 )的家庭护理需求以及与需求度相关联的因素进行调查及分析。方法 :以北京市 2 10例脑血管疾患老人为对象进行一对一的交谈。调查用的工具为朴贤泰 (2 0 0 2 )开发的一般健康状况评价工具和Beck er(1974 )感知的有益性工具 ,以及Kimsunsoon(1993)的家庭护理需求度测量工具。结果 :对家庭护理的感知的有益性在有配偶、学历较高、完全自费的情况下较高。患者本人的家庭护理需求与下列因素有关 :身体的活动功能越低 ,不适感越高 ,感知的有益性越高 ,家庭护理需求越高 ;照顾者的家庭护理与患者的社会功能成正比 :患者的社会功能越高 ,照顾者的家庭护理需求越高。而且 ,性别、配偶有无、面谈对象是患者本人或照顾者等因素都对家庭护理服务需求内容有差异。结论 :依据脑血管疾患老人对家庭护理的需求 ,确定我国家庭护理的服务对象、服务项目 ,以及宣传对象 ,从而开发出符合我国具体实际的家庭护理模式。Purpose: To identify the home care needs of the elderly patients in Beijing who were suffering from Cerebrovascular Diseases (CVD) as well as the affecting factors. Methods: The subjects of the study were 210 elderly people with CVD who were over age 60(or their caregivers).The measurement tools used in this study were based upon information from generic health status measurement by Hyuntae Park(2002), the perceived benefit by Becker(1974), and home care services by Sang Sun, Kim & Zae gue, Kim (1993). Results: The perceived benefit was significantly higher in patients who have spouse, with higher educational level and the subjects in own payment. Patients' home care needs were significantly higher with the following generic health factors: poor physical ability (ADL, IADL), high discomfort level and high perceived benefit. On the other hand, caregiver's home care needs were related with the following factors: the higher patient's social ability, the higher caregiver's home care needs. And the needs of home care services are significantly different by the gender、spouse、and the interviewer. Conclusion: It's of great need to establish an efficient and practical home care nursing system in China for CVD elderly.
