摘 要:目的 探讨慢性肾功能衰竭 (CRF)患者血清瘦素水平的变化及其与血红蛋白之间的关系。方法 以放射免疫分析法测定CRF患者血清瘦素的浓度 ,以全自动生化分析仪及常规分析方法测定肾功能和各有关的生化指标 ,以SPSS10 .0统计软件进行统计处理。结果 正常对照组血清瘦素浓度为 (10 .0 4± 7.0 0 ) μg L ,血清瘦素浓度与BMI呈明显相关 (r=0 .4 1,P <0 .0 1) ,且女性血清瘦素浓度较男性高 (t=3.194 6 ,P <0 .0 1)。CRF患者血清瘦素浓度仍然与BMI有关 ,其相关系数为 0 .4 5 (P <0 .0 1) ,男性患者血清瘦素浓度较女性患者低 (t=3.6 6 5 7,P<0 .0 1)。CRF男性较正常男性、CRF女性较正常女性其血清瘦素水平均显著升高 (t分别为 2 5 5 72和 2 916 6 ,P均 <0 .0 5 )。多元回归分析表明 ,血清瘦素水平受到血清白蛋白、血红蛋白、甘油三酯和GFR的影响 ,其回归系数分别为 1.2 0 2 1、1.4 36 6、2 .6 813和 - 0 .14 13,P均 <0 .0 5 ;而血红蛋白浓度受到血白蛋白、血清瘦素和尿素氮的影响 ,其回归系数分别为 2 .5 110、1.32 2 6和 - 0 .3112 ,P均 <0 .0 5。结论 本组CRF患者血清瘦素水平升高 ,不仅仅由GFR下降引起 。Objective To explore the relationship between serum leptin and renal anemia in patients with chronic renal failure (CRF).?Methods The concentration of serum leptin was determined by radioimmunoassay, the biochemical parameters were measured automatically or by routine methods. The data were processed for statistic analysis by using SPSS10.0 software.?Results The serum leptin level in normal controls was (10.04±7.00)μg/L and related to body mass index (BMI, r =0.41, P <0.01), being higher in males than in females [(6.96 ±5.29) μg/L vs. (12.87±7.21)μg/L, t =3.1946, P <0.01]. The leptin level was (9.98±6.32)μg/L and (18.83±11.26)μg/L in CRF males and females respectively (as compared to the normal controls, P <0.05). Multiple regression analysis showed the leptin level in CRF patients was influenced by the concentrations of serum albumin, Hb and triglyceride and the GFR, with the regression coefficients being 1.2021, 1.4366, 2.6813 and -0.1413 respectively ( P <0.05). The Hb concentration in CRF patients was influenced by serum albumin, leptin and BUN, with the regression coefficients being 2.5110, 1.3226 and -0.3112 respectively ( P <0.05).?Conclusion The serum leptin level in patients with CRF is elevated. This is not only a consequence of reduced GFR, but also a compensatory response to chronic anemia.
