摘 要:目的 探讨事件相关电位与智商及注意缺陷多动障碍 (ADHD)之间的关系 ,为诊治儿童智商和ADHD提供科学依据。方法 以DSM -IV诊断标准确定ADHD病例 60名 ,按年龄、性别 1∶1匹配正常对照组儿童 ,应用听觉oddball模式诱发事件相关电位 (ERP) ,采用韦克斯勒学龄儿童智力量表中国修订本 (WISC -CR)法进行智能测定 ,应用SAS统计软件对 2组儿童进行分析比较。结果 ADHD组事件相关电位计数正确率显著低于对照组儿童 ;P2 、N2 、P3 潜伏期显著延长 ;N1 P2 峰峰值显著低于对照组 ;2次记录P3 波重复性显著低于对照组儿童 ;智商 ,包括总智商 (FIQ)、言语智商 (VIQ)、操作智商 (PIQ)均显著低于对照组儿童。P2 、N2 、P3 潜伏期与FIQ、PIQ呈显著负相关 ;N1 P2 峰峰值与FIQ、VIQ呈显著正相关。交互作用分析结果显示 ,PIQ、VIQ对ADHD的直接影响是客观存在的 ,且N1 P2 峰峰值与VIQ之间明确地有着交互作用。结论 支持ADHD是由于额前区执行功能不成熟的病因学说。Objective To explore the relationship among event-related potential (ERP), intelligence quotient (IQ) and attention deficit hyperactivity disorder (ADHD).Methods Sixty children with ADHD were identified according to criteria for ADHD listed in the DSM-IV (the Diagnostic and Statistical Manual of Mental Disorders, Fourth Edition, American Psychiatric Association), an equal number of controls were 1:1 matched by age. ERP of all children was evoked with aural oddball paradigm, and their IQ was examined with WISC-CR (Wechsler Intelligence Scale for Children-Chinese Revised). Data were analyzed using SAS software.Results The overall percentage of correct responses measured by ERP was lower, P 2, N 2, P 3 latencies were found to be significantly longer, N 1P 1 wave amplitude was lower, repeatability of P 3 wave with duplicate measures was lower, and IQ including VIQ (Verbal Intelligence Quotient), PIQ (Performance Intelligence Quotient) and FIQ (Full Intelligence Quotient) reduced significantly in children with ADHD. P 2, N 2, P 3 latencies were correlated negatively with FIQ, PIQ and N 1P 1 wave amplitude and positively with FIQ, VIQ. Interaction analysis showed that there were direct reactions between PIQ and ADHD, and between VIQ and ADHD, while there were interaction between N 1P 1 wave amplitude and VIQ.Conclusion The findings support the hypothesis that ADHD is associated with a dysfunction of prefrontal brain regions.
