摘 要:目的 了解医院整改前后的胃镜使用与消毒状况 ,为卫生决策提供依据。方法 对合肥市辖区内所有开展胃镜诊疗的医院进行现场调查并采样 ,运用Logistic回归作单因素与多因素分析。结果 整改前共调查1 4所医院 ,消毒效果检测合格率为 30 .6 8% ,胃镜镜身消毒合格率为 1 6 .6 7% (3/ 1 8) ,活检钳灭菌合格率为 1 3.33% (2 / 1 5 )。整改后共调查 1 7所医院 ,消毒效果监测合格率显著上升为 96 .0 4 % ,胃镜镜身合格率为 94 .4 4 %(1 7/ 1 8) ,活检钳灭菌合格率为 1 0 0 % (1 7/ 1 7)。胃镜消毒效果单因素分析显示消毒、灭菌处理时间、三漕法、开展消毒剂浓度自我监测等因素与消毒效果显著相关。结论 合肥市胃镜消毒状况整改后与整改前相比有显著改善 ;运用三漕法清洗消毒胃镜 ,严格掌握消毒。Objective In order to know the present status of gastroscope disinfection in Hefei hospitasl before and after reforming, provide scientific basis for sanitary decision. Methods A survey and sampling of gastroscopes were carried out in 14 hospitals before reforming and 17 hospitals after reforming in Hefei. Logistic regression were used for analyzing the main risk factors of gastroscope disinfection. Results Before reforming, the qualified rate of sample was 30.68%, the surface of gastroscopes 16.67% (3/18) and biopsy nip 13.33%( 2/15), after reforming the rate was elevated to 96.04%, the surface of gastroscopes 94.44% (17/18) and biopsy nip 100% (17/17). Analysis by logistic regression showed that the main risk factors of gastroscope disinfection were the duration time of disinfection and sterilization, three-flume method and self-inspect for disinfectant concentration. Conclusions The gastroscopes disinfection improved significantly in HeFei hospitals after reforming. Three-flume method, duration time of disinfection and sterilization controlling was the key for gastroscope disinfection.
