带血管蒂拇展肌肌皮瓣局部转位术的临床应用

摘　　要：目的　根据应用解剖学基础 ,采用带血管蒂拇展肌肌皮瓣局部转位术治疗踝足部疾患。方法　对 11例患者实施带血管蒂拇展肌肌皮瓣局部转位术 ,其中内踝慢性骨髓炎 5例 ,足跟部溃疡 3例 ,跟腱部复合组织缺损 2例 ,足背软组织缺损 1例。结果　术后随访 6～ 11个月 ,肌皮瓣全部成活 ,且皮肤感觉恢复良好 ,无 1例发生皮肤破溃。结论　该肌皮瓣血供丰富 ,有多组血管蒂可选择 ,转位灵活 ,操作简便 ,可修复踝足部组织缺损。Objective To apply transposition of vascularized musculocutaneous flap of abductor hallucis muscle in treating affliction of malleolus and pedal on the basis of applied anatomy.Methods Transposition of vascularized musculocutaneous flap of abductor hallucis muscle was performed on eleven patients ,of those,5 were chronic osteomyelitis of medial malleolus,3 ulcer of heel,2 compound tissue defect of achilles tendon and 1 dorsal soft tissue defect of pedal.Results All musculocutaneous flaps survived completely,the patients were followed up for 6 to 11 months and dermal sensation recovery is good, there is no erosion in all cases.Conclusions The musculocutaneous flap has abundant blood supply and various vascular pedicel can be selected.The operation is convenient,the musculocutaneous flap is easy to transfer and can be used to repair tissue defect of malleolus and pedal.
