摘 要:目的 :研究螺旋 CT中耳仿真内镜的检查方法以及临床应用价值。方法 :对 1 0例正常及 1 5例耳部流脓、耳部外伤、外耳道闭锁和听力下降的患者进行容积扫描 ,层厚 1 mm,螺距 1 .0~ 1 .5 ,对两侧颞骨用高分辨骨算法小视野重建 ,重建间隔 0 .2~ 0 .3mm,利用 Navigator软件对容积数据进行三维重建 ,获得中耳仿真内镜图像。结果 :螺旋 CT中耳仿真内镜能清楚显示外耳道以及中耳腔内、外、前、后内表面 ,还可从不同角度观察听骨链 ,但是不能区分内表面密度差异。结论 :螺旋 CT中耳仿真内镜提供了一种新的方法 ,可以清晰地显示中耳的结构 。Objective:To investigate the technique and clinical value of spiral CT virtual endoscopy of middle ear.Methods:Volume scanning with a spiral CTunit was performed in ten normal cases and 15 cases with pyecchysis of external auditory meatus,aural injury,atresia of external auditory canal,or dysaudia.Slice thickness 1 mm and pitch 1 0 1 5 were selected as the scan parameters.The images of the right and left petrous bones were reconstructed separately using high resolution algorithm for bony structures with a small field of view and 0.2 0.3 mm reconstruct interval.The images were then put into a workstation and processed with the Navigator virtual endoscopic software. Results:A series of images were obtained as the virtual endoscopy moved from the external auditory canal to the middle ear cavity.Virtual endoscopy could well demonstrate the anterior,medial,lateral and posterior surfaces of the middle ear structures,including the ossicles.The disadvantage of the virtual endoscopy was that the density difference of the surfaces could not be differentiated.Conclusion:CT virtual endoscopy of the middle ear provided a new method of observing the anatomy of this complex region in detail.It had important value in surgical treatment.
