摘  要:目的:探讨脊髓损伤后远达效应的机理、病理及治疗方法。方法:家兔40只,通过改良Allen法制成脊髓损伤模型,截瘫按Talove分级为1级-2级,分为治疗组和对照组,治疗组于伤后3h开始自静脉给予常规剂量(0.35mg/kg)的地塞米松治疗,分别于不同的时相处死家兔,观察动物的心肺、胃肠、膀胱、肝肾等器官的病理改变。结果:①家兔脊髓损伤后均发生了不同程度的远隔脏器的损伤即远达效应,主要是肺出血(100%)。②脊髓损伤所致的远隔脏器的出血,治疗组明显轻于对照组(P<0.01)。结论:①脊髓损伤同机体其他部位损伤一样会发生远达效应;非高速投射伤也可引起远达效应。②远达效应的发生机理主要是神经-体液应急反应。③应用常规剂量的地塞米松即可以有效地预防和治疗脊髓损伤远达效应的发生。Objective: To study the mechanism, pathology and treatment of the remote effect after spinal cord injury. Methods: The models of spinal cord injury were made by methods of Allen in 40 rabbits and divided into group ① (control group) and group ② (experimental group). The group ② was treated with dexamethasone (0.35mg/k/d) by intravenous drip at first 3 days. The rabbits of spinal cord injury were killed to autopsy in different time and their heart, lung, stomach and bladder were observed. Results: ①,Remote effect, which showed hemorrhage of lung(100%), stomach, bladder may occurred after spinal cord injury.②, The hemorrhage of lung after spinal cord injury can be more lessened by the treatment of dexamethasone in group ②than that in group ① (P<0.01). Conclusion: ①, Spinal cord injury resulting from non- high-speed shooting injury may result in the remote effect.②, The mechanism of the remote effect was mainly nerve-body-fruids stress reaction. ③, Administration of dexamethasone in routine dosage may prevent and treat the remote effect after spinal cord injury.
