摘  要:目的 :探讨一期治疗前列腺增生 (BPH)伴膀胱结石的腔内手术方法。方法 :1998年 4月~ 2 0 0 2年 2月 ,采用经尿道前列腺汽化切除术 (TUVRP)和液电碎石 (EHL)治疗BPH伴膀胱结石 2 5例。结果 :碎石率为88% ( 2 2 /2 5)。平均手术时间为 77.6min。术中出血少 ,无膀胱穿孔。术后留置导管 3~ 5d。 90 .9% ( 2 0 /2 2 )患者拔管后排尿均较术前明显通畅 ;术后平均住院日为 6.0d ;术后 3个月随访 ,I -PSS均值从 2 2 .3分下降至6.4分 (P <0 .0 1) ;最大尿流率均值从 10 .5ml/s上升至 17.9ml/s(P <0 .0 1)。结论 :TUVRPd -EHL一期治疗BPH伴膀胱结石具有创伤轻微、疗效确切、安全可靠、并发症少、术后恢复快等优点 。Objective:To explore the methods of synchronou s transurethral operation for the patients of benign prostate hyperplasia (BPH) wi th bladder calculus.Methods:From April 1998, until February 2002, the opera tion of transurethral electrovaporization and resection of the prostate (T UVRP) and electrohydraulic lithotripsy (EHL)simultaneously were performed on 25 patients of BPH with bladder calculus.Results:The stone-shatter rate was 88% (22/25) and the mean oper ation time was 77.6 minutes (range 55~120 minutes).There were less bleeding in operation and no perforation of bladder took place. Catheter was pulled out 3~5 days after operation and 90.9%(20/22) patients urinated smoothly. Mean postoper ative hospital stay was 6.0 days. Three months after operation, the mean I-PSS w as d ecreased from 22.3 to 6.4 ( P <0.01)and the mean peak uroflow was increased from 10.5 ml/s to 17.9 ml/s ( P <0.01).Conclusions:The procedure of TUVRP and EHL synchronously for the treatment of BPH with bladder calculus has the virtue of less injury, safe effective, less complication, and shorter convalescent period and it should be considered one better choice for these cases.
