超声心动图对心肌梗塞的机械性并发症早期诊断的价值

摘　　要：应用超声心动描记术检测120例急性心肌梗塞(AMI)患者,发现伴有乳头肌功能不全及断裂18例,室壁瘤10例,室间隔穿孔及心室破裂各1例。表明超声心动描记术有助于早期发现AMI的机械并发症,并可对室壁瘤的大小、乳头肌功能不全所致的血液返流作出定量或半定量的估价,及观察到穿孔、破裂及断裂的部位及范围,为治疗及评价预后提供了依据。Eighteen cases of papillary muscle dysfunction or chordal ten-deninous fraction,10 cases of aneurysm,1 case of ventricular septal per-foration and 1 case of ventricular rupture were found in 120 patients with acute myocardiac infarction by echocardiographic examination.The results suggested that echocardiographic examination is a useful method to detect the mechanical complications of acute myocardiac infarction at early stage,to evaluate quantitatively or semiquantitatively the mitral regur-gitations caused by disfunction of papillary muscle,and especially to ob-serve directly the position,extent and size of perforations,ruptures and fractions.So it presented some of bases for therapy and evaluation pro-gnosis.
