自制同轴导管治疗输卵管阻塞性不孕症的临床应用研究

摘　　要：目的　探讨自制同轴导管系统治疗输卵管阻塞性不孕症的临床价值、疗效。方法　在DSA下 ,对 5 5例输卵管阻塞性不孕症采用自制导管进行选择性输卵管造影与再通治疗。术后常规口服抗生素一周 ,连续宫腔通液两个月经周期。结果　 5 5例中经常规输卵管造影发现 94条输卵管不通 ,其中 83条疏通 ,再通率 88.3 %。插管成功率为 97.9%。间质部与峡部阻塞复通成功率 92 .4% ,壶腹部阻塞复通成功率 75 .0 %。漏斗及伞部阻塞复通成功率 5 0 %。术后随访 6～ 12个月 ,受孕率 3 6.1%。未发生严重并发症。结论　用自制同轴导管系统行选择性输卵管造影与再通术操作简单、费用低 ,是一种安全、有效的治疗方法。Objective To explore the clinical value and effect on treatment of oviduct obstructive infertility with home made catheter. Methods Fifty five cases of oviduct obstructive infertility were performed selective salpingography and recanalization through DSA. After operation,antibiotic was taken regularly for 7 days and drug irrigation in uterine cavity was performed continually for two menstrual cycles. Results In 55 patients,94 oviducts were confirmed jam through regular salpingography. Of which 83 were successfully recanalized. The percentage of recanalization was 88.3%. The rate of successful catheterization was 97.9%.In uterine part and isthmus part,the rate of recanalization was 92.4%.In ampulla part,the rate of successful was 75.0%, and infundibular part and fimbria part was 50%. All cases were followed up for 6 to 12 months, the rate of pregnant was 36.1%, and no serious complication was not found. Conclusion Selective salpingography and recanalization are simple,cheap,safe,and effective procedure with micro catheter.
