特发性室性心动过速的心电生理观察和随访

摘　　要：报告8例特发性室性心动过速,其中持续性室速(≥30s)5例,非持续性室速(<30s)3例,各种检查未发现心脏扩大和瓣膜病。在窦性心律时,希氏束电图正常,心脏生理不应期在正常范围。电刺激右心室6例,其中2例诱发持续性室速,另外4例可诱发1～3个心室反应波,无1例出现室颤。出院后随访2年～5年,5例持续性室速有复发,3例非持续性室速有室性早搏,但均无心脏扩大,病情无恶化。Eight patients with idiopathic ventricular tachycardia were observed.There were 5 cases with sustained ventricular tachycardia(≥30s) and 3 cases with non-sustained ventricular tachycardia(<30s).The hearts were not enlarged on physical examination,X-ray and UCG.The cardiac elec-trophysiological parameters are as following:A-H internal is 94 ms,bundle of H is 16 ms and H-V internal 53 ms.The effective refractory periods of atrium and ventricle are 225 ms and 220 ms respectively.The sustained ven-tricular tachycardia could be induced by electrical timulation of right ventricle in 2 patients and 1 to 3 ventricular responses in 4 patients,and ventricular fi-brillation in none.The sustained and non-sustained ventricular tachycardia re-curred in 8 patients during follow-up period of 2-5 years,Their hearts were not enlarged and thcre were no murmurs on anscultation.
