摘  要:目的 学习后交通动脉的显微外科解剖特点。方法 借助手术显微镜在 6~ 2 5倍下对 15例 (3 0侧 )成人尸头进行解剖 ,对后交通动脉及其穿通动脉进行解剖与测量。结果 后交通动脉多发自颈内动脉床突上段的内侧壁或下内侧壁 ,长度和外径分别为 (14 .0 3± 0 .3 6)、(1.82±0 .5 9)mm ,每侧后交通动脉发出 (6.69± 0 .3 2 )支穿通动脉 ,经视束与灰结节的间隙、灰结节、旁穿质、漏斗和乳头体进入脑内。结论 后交通动脉相对恒定地向三脑室底 下丘脑发出穿通动脉 ,在后交通动脉瘤夹闭等显微手术中应注意对它们识别和保护。Objective To study the microsurgical anatomic feature of the posterior communicating artery (PcoA).Methods The PCoA and its perforating arteries was examined in 15 formalin fixed adult cadaveric heads in which the arteries were perfused with red silicone.A 6 to 25 power neurosurgical operating microscope was used for all dissection and measurement.Results The PCoA artery taken origin from the infero lateral wall of the supraclinoid internal carotid artery within a few millimeters of the anterior portion of the carotid cistern with a mean length of (14.03±0.36) mm and a mean outer diameter at its origin of (1.82±0.59) mm.The average number of perforating arteries from PCoA was (6.69± 0.32) including the premamillary artery,the most constantly occurring and largest branch.Those vessels penetrated the hypothalamus region at the tuber cinereum,the sulcus between the optic tract and the tuber cinereum,the paramedian perforated substance,the circumin fundibular,and the mamillary body.Conclusion In the microsurgical management of lesions in the sellae region,a thorough knowledge of the microsurgical anatomic features and microsurgical techniques of the PCoA is vital,and the vessle and its perforating arteries should be identified and manipulated carefully.
