摘 要:目的 探讨分娩状态下胎盘滋养层细胞bcl-2、Fasl的蛋白表达变化及其与母血、羊水中表皮生长因子(EGF)、内皮素(ET)的变化之间的关系。方法 应用免疫组化法测定53例足月妊娠产妇胎盘滋养层细胞bcl-2、Fasl的蛋白表达,用放射免疫法测定产妇母血、羊水中EGF、ET的含量。其中临产组28例;未临产组25例。结果 (1)临产组胎盘滋养层细胞bcl-2、Fasl的蛋白表达明显低于未临产组,两组平均灰度及阳性细胞率比较,差异有显著意义(P<0.05)。(2)两组母血、羊水中EGF、ET水平均高于正常值,且临产组高于未临产组,差异有显著意义(P<0.05)。(3)母血、羊水中EGF、ET的变化与bcl-2的蛋白表达呈负相关,而与Fasl的表达不相关。EGF、ET浓度与bcl-2的平均灰度及阳性细胞率均呈负相关,相关系数r检验差异有显著意义(P<0.05)。结论 胎盘凋亡基因bel-2、Fasl介导的滋养层细胞凋亡在启动分娩发动过程中具有重要作用;bcl-2的作用可能受EGF、ET等细胞因子的调控。Objective To investigate the expression of bcl-2 and Fasl proteins in placental trophoblastic cells during the course of labor and their relationship with Epidermol Growth Facterc(EGF) and Endothelins (ET) in maternal blood and amniontic Fluid. Methods Fifty-three women on term pregnancy were divided into two groups. One group in the time of labor onset were twenty-eight women. Another group not on the time of labor onset were twenty-five women. Expression of bcl-2 and Fasl proteins in placental trophoblastic cells were detected by immunohistochemical assay using monoclonal antibody. EGF and ET in maternal blood and amnionotic fluid were detected by radioim-mune assay. Results (1) Expressions of bcl-2 and Fasl proteins in group labor onset was significantly lower than the group not labor onset. There was a statistically difference in the average gray and the ratio of positive cells between the two groups (P<0. 05). (2) The concentrations of EGF and ET in maternal blood and amnionotic fluid of two groups were higher than nomal. But the concentration in labor onset group were higher than not labor onset (P<0. 05). (3)There was a negative correlation between the expression of bcl-2 proteins and the concentrations of EGF and ET in maternal blood and amnionotic fluid. But it was not fitted to Fasl. The average gray and the ratio of positive cells of bcl-2 had a negative correlation with the concentrations of EGF and ET respectively. There was a statistically difference in correlation index (r) (P<0. 05). Conclusion It was considered that the apoptosis of placental trophoblastic cells induced by bcl-2 and Fasl may play an important role in the initiation of term labor onset. EGF and ET may control the expression of bcl-2 by influencing their low-stream of genie transcriptions.
