摘　　要：目的:探讨儿童股骨头无菌性坏死(Legg-Perthes病)的病因与预后。方法:回顾性分析近30年收治75例单侧Perthes病骨盆X线影像变化的表现形式及发生率。结果:髋骨外旋畸形的发生率为70％,髂骨水平横径增宽、坐骨棘内突粗长、闭孔横径缩小。髋骨发育不良发生率为54％,耻坐支细小、耻坐弓底缘上提、弓状线不规则改变、髂嵴高度下移。髋臼异常发生率为15％,臼面不规则、Y形软骨密度不均。同侧股骨干细小发生率为4％。结论:Legg-Perthes病并非单一股骨头的病理改变,可能是儿童生长期骨骺发育异常疾病的系列表现;骨盆X线影像变化越严重,预后则越差。Objective: To study the etiology and prognosis of Legg-Perthes disease. Methods: Pelvic X-ray images of 75 cases of Legg-Perthes disease in the past 30 years were reviewed, the change pattern and incidence of pelvic X-ray images were described, the relations between the serious changes of pelvic X-ray image and the prognosis were analyzed. Results:The incidence of hip bone turning outward was 70% .showing the increasing of two iliums cross width; the stretching inward of ischial spine, the decreasing of obturator foramen cross width. The incidence of hip bone dysplasia was 54% ,showing the thinness of ischium and pubis branches, the upward lifting of ischium and pubis branches, the unregular change of arcuate line and the decreasing of iliac crest height.The incidence of acetabulum dysplasia was 15% .showing the unregularity of lunate surface and undensity of acetabulum cartilage. The incidence of thinness of collateral femur trunk was 4% . Conclusion: Legg-Perthes disease has not only one pathology change in femur head,and probably may have lots of epiphyseal dysplasia in child developmental period. There is greatly relationship between the change of pelvic X-ray image and prognosis.
