颅面复合伤42例的临床救治

摘　　要：目的 :总结重型颅面复合伤 4 2例的临床救治经验。方法 :诊治重型颅面复合伤 4 2例 ,男 30例 ,女12例。开放性伤口手术 2 5例 ,闭合性伤经双眉弓至鼻根部联合整修 2例 ,行面中部掀翻术 1例 ,柯陆氏径路手术 13例 ,面中至颞侧双径路手术 1例。结果 :除 1例面瘫外 ,4 1例痊愈出院。结论 :受伤患者伴有不同程度的系统损伤时 ,先确定有无颅脑损伤 ,喉、气管损伤 ,胸腹损伤及外伤性休克等 ,争分夺秒地急救处理。处理鼻额筛眶的原则是恢复鼻功能、视觉功能和咬牙合功能 。Objective:To summarize clinical therapeutic experience of patients with serious craniofacial injuries.Methods:42 patients (male 30,female12) with serious craniofacial injuries had been treated.25 cases had been operated through open wound; 2 cases with closed wound had been performed combined renovation via superciliary arch and nasal root; one case had been operated by turning center of the face open; 13 cases had been practiced Caldwell-Luc operation; one case had been performed associated operation via center of face and temple so as to avoid complications. Result: All the patients were cured except one case with the complication of facial paralysis. Conclusion:For the injured patients with various system damages, first of all ,it must be defined whether there exit craniocerebral injury, lesions of larynx and trachea, injuries of chest and abdomen and traumatic shock which must be treated immediately. Recovery of nasal function, visual function and bite function is the principle in the treatment of the injuries of nose, brow, ethmoid sinus and orbit. Meanwhile, the appearance and cosmetology must be considered.
