摘　　要：目的　探讨大鼠原位肝移植实验的围手术期处理的特点。方法　在Kamada用“二袖套法”吻合血管的基础上进行改良。供体改经腹主动脉进行肝脏冷灌注 ,肝上下腔静脉用缝合法吻合 ,门静脉和肝下下腔静脉用袖套法吻合 ,胆总管采用单管内支架胆管端吻合法。结果　共施行大鼠原位肝移植 36 0次 ,手术成功率为 91 3%。非干预组中同系移植大鼠 1周存活率为 86 5 % ,3个月存活率达 80 7%。结论　积极手术前准备 ,手术中熟悉步骤、主动配合、手术后耐心细致护理、密切观察、可以保证大鼠原位肝移植实验顺利进行、减少并发症的发生、提高动物的长期存活率。Objective To investigate the surgical perioperative management of establishing a rat model of orthotopic liver transplantation.Methods The model was established with modified cuff technique. The donor was perfused through the abdomial aorta with 20 ml cold perfusate. The anastomosis of the suprahepatic vena cava (VC) was sutured end-to-end with 8-Onylon line, and continuity of infrahepatic VC and portal vein(PV) was established by means of cuff method respectively. The bile duct anastomosis was performed by internal stent. Results Three hundred sixty cases of rat orthotopic liver transplantation were performed, and the successful rate of the mode was 91.3%. In non-intervention group, the survival rats were satisfactory, with a 86.5% of 1-week survival and a 80.7% of long-term survival (longer than 3 months).Conclusion Orthotopic liver transplantation in rats can be performed smoothly, the complication reduced largely, and the long-term survival raised under positive preoperative preparation,familiarity with operative procedure, and intensively post-operation nursing and observating.
