AQ与CK技术对冠状动脉成形术前后左心室容积与收缩功能的研究

摘　　要：目的 用声学定量(AQ)、彩色室壁运动技术(CK)评价经皮冠状动脉成形术(PTCA)前后左心重容积及收缩功能变化。方法 对33例择期行PTCA+支架术的冠心病患者用HP5500型超声诊断仪于心尖四腔心观对左室行AO检测,并于胸骨旁乳头肌短轴观的前间壁、下壁、后壁、侧壁行CK检测,分析PTCA术前及术后3天、1-3个月时的左心室舒张末期容积及收缩功能的变化。结果 AQ所检测的左室射血分数、左室舒张末期容积和CK所检测的室壁运动幅度等在术后3天有所改善(但无统计学意义,P>0.05),术后1-3月检测,左心室容积及收缩功能均有明显改善(p<0.01)。结论 PTCA能有效地挽救存活心肌,AQ和CK技术能够较准确评价PTCA疗效,能为临床提供新的有价值的评价指标和信息。Objective To evaluate left ventricular volume and systolic function before and after PTCA by acoustic quatification and color kinesis technique. Methods Left ventricle was detected at apical four chamber view by AQ and the anterior wall , inferior wall , posterior wall, and lateral wall were detected at parasternal papillar muscle level left ventricle short axis view by CK in 33 cases of patients with coronary heart disease , operated by PTCA + stent at selected time and analysed changes of left ventricular end diastolic volume and systolic function before PTCA, 3 days and 1-3 months after PTCA. Results EF、EDV measured by AQ, wall motion extent measured by CK had a little improved at the 3th day after PTCA( but had not apparent statistical significant, P > 0.05) , left ventricular volume and systolic function all had improved at the follow - up study (P<0.01). Conclusion PTCA can effectively save viable myocardium. AQ and CK can much accurately evaluate PTCA together, which can provide valuable indexs and messages.
