摘  要:目的 观察研究糖尿病胃动力障碍 (DGMD)发生的电生理机制及细胞病理学基础。方法 清洁级雄性SD大鼠胃窦部埋植银电极 ,以成都泰盟BL -4 10型生理实验系统记录胃电。除对照组大鼠外 ,其余大鼠腹腔注射链脲佐菌素以制作糖尿病模型 (DM组 ) ,并随机抽取约一半造模大鼠灌服中药复方“糖胃康”(DM -TCM组 )。实验电生理观察 2个月后 ,3组大鼠的胃组织作纵肌 -神经丛铺片 ,并综合应用NDP组化染色、ZIO组化染色、c -kit及nNOS免疫组化法 ,以光镜及电镜观察DM组大鼠胃动力基本功能单位 (BFUGM) ,即壁内神经 (ENS) -Cajal间质细胞 (ICC) -平滑肌(SMC)的病理改变 ,并观察“糖胃康”的防治作用。结果 DM组大鼠的胃电明显异常 ,主要表现为胃电基本电节律的频率、波幅及峰电发生率均低于对照组 ,其波幅尚随造模时间推移而下降。而DM -TCM组大鼠胃电波幅随造模时间推移的下降则较轻微 ,其峰电发生率亦高于DM组。DM组大鼠胃动力基本功能单位有明显的病理改变 ,其SMC排列紊乱 ,SMC内有较多胞质溶解性空泡 ,线粒体肿胀 ;肌间丛ENS神经元胞质部分溶解 ,线粒体肿胀 ,嵴溶解 ,神经轴突有较多空泡性变 ;ICC分布不均 ,数量减少 ,线粒体肿胀 ,核周细胞浆减少等。DM -TCM组大鼠胃ENS、ICC及SMC的病理改变明显轻于DM组。Objective To reveal the electrophysiological manifestation and cytopathological basis of diabetic gastromotility disorder(DGMD). Methods Male Sprague-Dawley rats of clean class(CL) were used for the study. Silver electrodes was embedding in gastric antrum in rats and 'Chengdu Taimeng BL-410 physiological experimental system' was used to record electrogastric activity. Streptozotocin was injected intraperitoneally to establish diabetic models. Diabetic rats were randomly divided into diabetes model group(DM group) and diabetes model+TCM group(DM-TCM group). DM-TCM group was treated with traditional Chinese medicine 'tangweikang'. After 2 months of electrophysiological observation, rats were killed and stomach tissues were examined for ZIO, c-kit and nNOS by immunohistochemistry. Light and electron microscope were used to study the pathological changes of basic functional unit of gastric motility(BFUGM), i.e. enteric nervous system (ENS)-interstitial cells of Cajal (ICC)-smooth muscle(SMC) in rats of DM group and to observe the preventive effect of 'tangweikang'. Results ①There were apparent abnormal electrogastric activities in rats of DM group. The frequency and wave amplitude of basic electric rhythm(BER) of electrogastric activity and the incidence of spike potential were lower than that control rats. The wave amplitude decreased with time in DM model. ②The decrease of BER amplitude of electrogastric activity in rats of DM-TCM group with time in DM model making was slight and the incidence of the spike potential was also higher than that of DM groups. ③There were marked pathological changes of BFUGM in rats of DM group, including disorganized arrangements, cytoplasmic dissolving vacuoles and swelling mitochondria in SMC; partial resolution of cytoplasm, swelling mitochondria, lysis crest, and vacuolar changes in nerves axon of neuron of the intermuscular nervous plexus; uneven ICC distribution, swelling mitochondria and diminished cytoplasm around the nucleus. ④The pathological changes of BFUGM in ra
