老年充血性心力衰竭患者血浆甲状腺激素和脂蛋白(a)的变化

摘　　要：目的　研究充血性心力衰竭 (CHF)患者血清甲状腺激素与Lp(a)水平变化的关系。方法　以 5 0例正常人为对照 ,15 8例CHF患者按心衰病程分为病程 <1年组和病程≥ 1年组。测定血清FT3、FT4 、TSH、Lp(a)水平。结果　心衰病程 <1年者FT3低于正常人 (P <0 0 5 ) ,Lp(a)高于正常人 (P <0 0 5 )。心衰病程≥ 1年组 ,其血清FT3明显低于正常人组 (P <0 0 5 ) ,亦低于CHF病程 <1年组 (P <0 0 5 ) ;其血清Lp(a)水平高于正常人组 (P <0 0 1) ,亦高于心衰病程 <1年组 (P <0 0 5 )。结论　心衰患者Lp(a)水平升高 ,随着心衰病程延长 ,其血清Lp(a)水平逐渐升高 ,血清活性甲状腺激素FT3水平逐渐降低。推测血清活性甲状腺激素FT3水平下降 ,影响了Lp(a)的代谢 ,是导致心衰患者Lp(a)水平升高的原因之一。Objective To investigate the relationship between thyroid hormones and Lp(a) the pituitary-thyroid axis in elder patients with congestive heart failure(CHF). Methods 50 normal controls and 158 elder patients with CHF who had not thyroid gland illness were involved in this study. Levels of FT 3, FT 4, TSH and Lp(a) in serum were measured. Patients were divided into two groups according to the course of chronic heart failure: patients with the course of CHF <1 year and patients with the course of CHF≥ 1 year. Results Levels of FT 3 in the course of CHF<1 year group were lower than those of normal control group(P<0 05),but levels of serum Lp(a) were higher than those of normal control group(P<0 05) . Serum levels of FT 3 in patients with the course of CHF≥1 year group were significantly lower than those of normal control group(P<0 01) and those of patients with the course of CHF<1 year group(P<0 05), but serum levels of Lp(a)were higher than those of normal control group(P<0 01) and those of patients with the course of CHF<1 year group(P<0 05).Conclusions The findings suggest that serum levels of thyroid hormones are decreased but serum levels of Lp(a) are increased in patients with CHF,especially in patients with the course of CHF≥ 1 year group. The increase of the serum levels of Lp(a) in patients with CHF can be explained at least partially by the decrease of the serum levels of thyroid hormone FT 3.
