摘  要:目的 :探讨二袖套法大鼠原位肝移植的手术技巧及并发症的预防。方法 :在Kamada“二袖套法”基础上进行改良。获取供肝前阻断肝门血供 10min ,再灌注 10min ;分别经腹主动脉和门静脉对肝脏进行双重灌洗 ;门静脉和肝下下腔静脉用袖套法吻合 ,肝上下腔静脉用缝合法吻合 ,胆总管采用单管内支架胆管端端吻合法。结果 :共施行大鼠原位肝移植 12 0次 ,手术成功率为 90 .8%。平均无肝期为 (2 1± 3.5 )min ,受体总手术时间为 (4 6± 4 .5 )min ,一周生存率为 87.2 %。结论 :良好的手术野暴露。Objective:To investigate the surgical technique of orthotopic liver transplantation in situ using two cuff technique and prevention of operational complication in rats.Methods:The model was established with modified cuff technique.Before donor livers were harvested,the portal vein and hepatic artery were interrupted for 10 min,and reflow was initiated for another 10 min.The donor liver was perfused through abdominal aorta and portal vein respectively.The infrahepatic vena vein(VC) and portal vein(PV) were established by means of cuff method;the anastomosis of the suprahepatic VC was sutured end to end with a 7 0 nylon line.The bile duct anastomosis was performed by internal stent.Results:One hundred and twenty cases of the rat orthotopic liver transplantation using two cuff technique were performed and the successful rate of the model was 90.8%.The average nonhepatic time of recipients was(21±3.5)min and the total surgical time was(46±4.5)min.The one week survival rate of recipients was 87.2%.Conclusions:Good exposure of operation field,sophisticated microsurgical technique and delicate surgical manipulation can decrease nonhepatic period and total surgical time of recipients and increase the survival rate of recipients.
