摘 要:目的 :探讨CR与螺旋CT对诊断脊柱爆裂性骨折的不同价值。方法 :对 58例脊柱爆裂性骨折进行回顾性分析。结果 :58例患者椎体受损总数 67个 ,经螺旋CT诊断为爆裂性骨折 58例 62个椎体 ,经CR诊断为爆裂性骨折有 53例54个椎体 ,对爆裂性骨折的检出率螺旋CT为 92 .65% ,CR为 80 .59% ,χ2 =4.11,P <0 .0 5;PVBL异常的显示率CR为91 3 8% ,螺旋CT为 77.59% ,χ2 =4.2 1,P <0 .0 5;对骨性椎管狭窄的显示率螺旋CT与CR总体相近 ,但CR对软组织病灶引起的脊髓受压显示尚不满意。结论 :CR对脊柱损伤的空间分辨率较高 ,螺旋CT对脊柱爆裂性骨折的确诊、骨折稳定性和脊髓受压情况的判断等占优势 。Objective:To explore the differ ent value of CR and SCT on the dia gnosis of spinal bursting fracture.Methods:58 Cases of diagnosis of spinal bur sting fracture were reviewed.Results:The total injured vertebra was 67 in 5 8 cases, the number of vertebra with diagnosed bursting fracture by SCT was 62(5 8 cases) and the number of vertebra with diagnosed bursting fracture by CR was 5 4(53 cases).The detected rate of bursting fracture of SCT was 92.65% and which o f CR was 80.59%,χ 2=4.11,P<0.05.The indicated rate of abnormal PVBL was 91.38% by CR and the same type rate was 77.59% by SCT,χ 2=4.21,P<0.05. Th ough the indicated rate of bony spinal canal stenosis of SCT was nearly close to that of CR, the CR could not be good at determining that compressed spinal cord by soft tissue lesions.Conclusion:The app lying of CR was superior in the spat ial resolution of spine trauma but the SCT scanning had much advantage in some c ases,such as the definite diagnosis of spinal bursting fracture,the estimation o f the fracture stability , the estimation of the degrees of compressed spinal cor d and so on.The CR combined with SCT could be helpful to improve the synthetic d iagnostic level of spinal bursting fracture.
