摘 要:目的 :探讨实时三维超声心动图在心血管疾病中的诊断价值。方法 :在 1 2例正常人和 45例心脏病患者中进行实时三维超声心动图检查 ,其中 2 0例患者与手术结果进行比较。结果 :1 .实时三维超声心动图可实时显示心脏的立体结构 ;2 .完整显示二尖瓣、三尖瓣及主动脉瓣的三维图像 ,尤其在瓣膜的短轴观方位显示瓣叶的位置、数目、活动度及狭窄口的最小面积更为清晰 ,评价人工瓣的功能状态 ;3.可实时显示室间隔或房间隔缺损的空间位置、形态 ,测量缺损最大直径与手术结果对照差异无统计学意义 (P>0 .0 5) ;4.在法乐氏四联症患者显示病变的复杂结构 ,尤其在主动脉根部的短轴观方位显示室间隔的骑跨程度 ;5.在心室多发粘液瘤患者 ,可立体显示粘液瘤的数目、空间位置关系、活动状态 ;6.显示血管壁的立体结构、斑块的位置、形态及活动度。结论 :实时三维超声心动图能够实时显示心脏的立体结构及功能状态 。Objective:To investigate the clinical a pplication of real time three dimensional echocardiography(RT 3DE) in patien ts with cardiovascular diseases Methods:Two dimensional echocardiography (2 DE),RT 3DE and full volume three dimensional echocardiography (FV 3DE) were performed in 12 normal volunteers and 45 patients with various heart diseases,in whom the re sults of 20 patients derived by echocardiography were compared with those of ope rations Results:Compared with conventional 2 DE technique,RT 3DE and FV 3DE had the advantages in following aspects:instantly depicting three o rthogonal planes and spatial relationships of any cardiac structure; revealing t he whole views and motion of mitral,tricuspid and aortic valves in valvular dise ases,especially the smallest area of valvular stenosis in short axis orientation ; desplaying location and morphology of atrial or ventricular septum defects ins tantly,without significant difference between dimensions of atrial or ventricul ar septal defects derived by 3DE and surgical operation( P >0 05);observin g complex deformation of Fallot tetralogy,especially overiding of ventricular se ptum in aortic short axis orientation; desplaying spatial relation of multiple m yxoma in biventricle;stereoscopically revealling constructures of vascular walls and plaques Conclusions:The real time three dimensional echocardiogr aphy provides a reliable technique displaying cardiac structures and function n ot available by 2 DE,which may enhance the qualitative and quantitative diagnos tic accuracy and work efficiency of echo laboratories and produce potential clin ical values for cardiovascular diseases
