彩色多普勒超声在颈动脉狭窄血管内支架成形术中的应用价值

摘　　要：目的 :探讨彩色多普勒超声在颈动脉狭窄血管内自膨胀支架治疗中的作用。方法 :2 3例临床表现为反复的短暂性脑缺血发作 (TIA)或脑梗塞患者 ,彩色多普勒超声诊断为颈动脉狭窄 ,超声检查内容包括 :颈总动脉和颈内动脉内径、最狭窄处内径、最大血流速度 (Vmax)、阻力指数 (RI)、搏动指数 (PI)、斑块的大小 ,计算狭窄的程度。经数字减影血管造影 (DSA)检查并行自膨胀支架置入成形术治疗 1 7例 ,术后 1周和 5～ 1 3个月(平均 9.3个月 )复查上述指标 ,并与 DSA对照。结果 :超声诊断颈动脉狭窄的总准确率为 70 .97% ,超声测量颈动脉内径和狭窄程度与 DSA测值比较 ,无显著性差异 (P>0 .0 5) ,术后超声显示支架定位准确 ,与术前相比 Vmax减低、RI降低、PI无明显变化。超声及 DSA随访 1 0例患者均无再狭窄。结论 :彩色多普勒超声对颈动脉狭窄的诊断准确性较高 (有无狭窄和狭窄程度的判断 ) ,可用于血管内支架成形术治疗颈动脉狭窄的初选。Objective:To evaluate the role of col or Doppler ultrasonography in endovascular therapy of carotid stenosis with self expandable stents.Methods:Twenty three patients who expe rienced repeated transient ischemic attacks or cerebral infarction were diagnose d as carotid stenosis by color Doppler ultrasonography The diameter、blood str eam parameters and degree of stenosis were measured Every one was examined by D S A and 17 cases with carotid stenosis were inplanted with self expandable stents .One week and 5～13 months after operation,above parameters were repeated by ul trasonography and compared with DSA.Results:The diagnosis accurate rate in color Doppler ultra sonography was 70 9%.The diameter and the degree of carotid stenosis measured b y ultrasonography had no significant difference with DSA( P >0 05) After operation,the self expandable stents were well localized and showed by ultra sonography.Vmax and RI were lower than those before operation and no restenosis was found in follow up.Conclusions:Carotid stenosis could be diagnosed accuratel y by color Doppler ultrasonography It was of great significance in initial sele ction,effect assessment and follow up in the endovascular therapy of corotid s tenosis with self expandable stent.
