摘 要:目的 通过MRI研究非脊柱脊髓畸形的国人人群圆锥位置的变化。方法 应用 1.5TMRI机对 1199例病人行腰椎MR扫描 ,自旋回波T1加权、正中矢状位像上观测其圆锥末端的位置。根据其与相应椎体的上、中、下 1/3及椎间盘的对应关系定位。结果 圆锥末端的平均位置为L1的下 1/3水平 (范围 :T12 的中 1/3~L3 的上 1/3 ) ,圆锥末端的位置呈正态分布 ,女性圆锥位置低于男性 (P <0 .0 0 1) ,各年龄段无差别 (P >0 .0 5 )。结论 MRI可以观测到圆锥末端的位置 ,对临床椎管内麻醉、腰椎管手术和椎管造影有重要参考意义。Objective To determine the variation in position of the conus medullaris in the Chinese population without spinal deformity through MR imaging. Methods The T1 weighted, midline, sagittal, spin echo magnetic resonance imaging studies of 1199 patients were assessed to identify the tip of the conus medullaris by using a 1.5T MRI machine. The location was recorded in relation to the upper, middle, or lower third of the adjacent vertebral body or the adjacent intervertebral disc. Results The mean conus position was at the lower third of L1 (range: middle third of T12 to upper third of L3). The variation in conus positions followed a normal distribution. The conus position in female patients was significantly lower than that in male counterparts ( P <0.001). No significant difference in conus position was seen with increasing age ( P >0.05). Conclusion MRI can show the distribution of conus location ranged from the middle one third of T12 to the upper one third of L3, and it is of significant importance to clinical anesthesia and operation.
