Ⅳ型胶原和层粘蛋白与甲状腺功能联合检测的临床应用

摘　　要：目的　了解甲亢患者基底膜代谢变化以及Ⅳ型胶原 (Ⅳ C)和层粘蛋白 (LN)在甲状腺功能亢进 (甲亢 )与其它甲状腺疾病鉴别诊断中的价值。方法　应用电化学发光免疫分析测定 194例各种甲状腺疾病患者的Ⅳ C、LN及甲状腺功能水平 ,并以 35例正常人作为对照。结果　未经药物治疗的 92例甲亢患者的Ⅳ C及LN水平均高于正常对照组和其他甲状腺疾病 (P <0 .0 1)。而 4 3例甲亢患者接受抗甲状腺药物治疗 ,完成疗程且甲状腺功能恢复正常者 ,其治疗前后的Ⅳ C及LN水平差异有非常显著性 (P <0 .0 1) ;2 6例甲状腺机能减退 ,2 5例单纯性甲状腺囊肿 ,8例甲状腺滤泡腺癌患者与正常对照组比较 ,Ⅳ C及LN水平则差异无显著性 (P >0 .0 5 )。结论　甲亢患者基底膜代谢有显著变化 ,ⅣObjective To understand the metabolic changes of the thyroids basemental membrane and the value of collagen Ⅳ(Ⅳ C) and laminin(LN) in the diagnonis of hyperthyroidism and other thyroid illness.Methods The level of thyroid gland function and Ⅳ C,LN were detected with ECLIA in 94 patients suffered from various thyroid illness,and the results were compared with that of 35 normal people.Results The levels of Ⅳ C and LN in 92 patients with hyperthyoidism(who had never received any treatment)were obviously higher than those in the normal people and patients suffered from other thyroid illness( P <0.01).However,the levels of Ⅳ C and LN in 43 patients with hyperthroidism,whose thyroid gland function had returned to normal after treatment of anti thyroid medicine and finished the course of treatment,were also quite different from those before treatment( P <0.01).The levels of Ⅳ C and LN in 26 patients with hypothyroidism,25 patients with simple thyroid cyst and 8 patients with thyroid follicular adenocarinoma,were the same as those in the normal controls( P >0.05).Conclusion The metabolic changes of thyroid basemental membrane in patients with hyperthyroidism is obvious and the combined inspection of Ⅳ C and LN can be regarded as new of composite critercia for hyperthyroidism to evaluate the condition of thyroid gland funciton.
