摘　　要：目的　探讨肿瘤坏死因子 (TNF)启动子基因多态性与急性胆原性胰腺炎及其脓毒症休克的关系。方法　该研究包括 12 7例急性胆原性胰腺炎 (轻型 6 6例 ,重型 6 1例 )患者 ,对照组为 10 2例正常志愿者。多态性检测是利用PCR扩增目的基因片段 ,用NcoI内切酶消化酶切位点 ,聚丙烯酰胺凝胶电泳分析酶切产物显示TNF2和TNF1。检测血浆TNF -α浓度用酶联法。结果　TNF2出现的频率在急性重症胆原性胰腺炎 (ASBP)组和急性轻型胆原性胰腺炎 (AMBP)组无显著性差异 ,在急性胆原性胰腺炎 (ABP)组和对照组也无显著性差异。TNF2出现的频率在脓毒症休克组明显高于无脓毒症休克组 ,其差异具有显著性。TNF -α - 30 8各核型在ABP组和对照组、ASBP组和AMBP组出现的频率均无显著性差异。TNF2与血浆TNF -α浓度无相关性。结论　TNF2与ABP的易感性及严重程度无关 ,与血浆TNF -α浓度也无相关性。TNF2能提高ASBP患者脓毒症休克的易感性。Objective To investigate TNF-α-308 polymorphisms in acute biliary pancreatitis (ABP) and septic shock from it.Methods Genomic DNA was prepared from peripheral blood leukocytes.Genotypes and allele frequencies were determined in patients (n=127) and healthy controls(n=102) using restriction fragment length polymorphism analysis of polymerase chain reaction (PCR) products.Reading the size of digested bands from polyacrylamide gel demonstrated the two alleles TNF1 and TNF2.Results The frequencies of TNF2 polymorphism was similar in patients with mild or severe pancreatitis,so were in pancreatitis patients and in controls.Patients with septic shock showed a significantly higher prevalence of the TNF2 than those without.No significant differences were found in the genotype distribution of TNF-α-308 among different groups.Plasma TNF-α levels did not differ significantly in ASBP patients displaying different alleles of the TNF gene studie.Conclusions Results indicated that TNF gene polymorphisms studies played no part in determination of disease severity of susceptibility for acute biliary pancreatitis;however,TNF2 was associated with septic shock from ASBP.Genetic factors were not important in determining plasma TNF-α levels in ASBP.
