恬尔心治疗冠状动脉旁路移植术后高血压的临床观察

摘　　要：目的 观察恬尔心治疗冠状动脉旁路移植术(CABG)后高血压的临床效果。方法 选择37例CABG术后半小时出现高血压的患者,首剂恬尔心0.2mg/kg,生理盐水稀释到10ml,3min内静注完毕,然后以5～15μg/kg·min^(-1)的剂量微泵持续给药,观察给药前后心率、血压、心电图的变化。结果 37例患者首剂注射完毕后,血压即出现下降,用药后1min时血压显著下降(P<0.05),2min时血压降低更加明显(P<0.01),15～30min左右血压下降至较为理想的水平(SBP<140mmHg,DBP<80mmHg)。结论 恰尔心治疗CABG术后高血压起效快,降压作用明显,对心率影响小,无明显不良反应。Objective To observe the effect of diltiazem on postoperative hypertension after CABG. Methods 37 patients were charged into ICU because of hypertension half an hour after CABG. All of the patients received diltiazem 0.2 mg/kg in 3 minutes as a loading dose and then were continuously infusion by 5 - 15 μg /kg · min-1 with micro syringe. HR, BP, ECG were monitored. Results The blood pressure of all the patients declined just after diltiazem were given, and declined obviously after 1 min, declined more obviously after 2 minutes, declined to an ideal level after 15-30 minutes and kept a stable level in 30 -60 minutes. During the observation, HR and ECG didnt change obviously. Conclusion Diltiazem has good effect on postoperative hypertension.
