两种浓度异氟醚低流量洗入半紧闭环路麻醉的临床观察

摘  要:目的 :观察两种浓度异氟醚低流量洗入半紧闭环路的摄取规律 ,评价临床应用异氟醚低流量洗入的可行性。 方法 :选择 30例ASAⅠ~Ⅱ级择期行上腹部手术的病人 ,随机分为A、B两组 ,两组洗入期新鲜气体流量均为 :氧气 1L/min ,异氟醚蒸发器设定浓度分别为 3%、5 %。维持期新鲜气体流量和蒸发器设定浓度均不变 ,预设目标浓度呼气末异氟醚体积分数 (FA)为 1.2 %。 结果 :A、B两组达到目标浓度的时间分别为 (4± 1)min、(16± 3)min(P <0 .0 5 ) ,6 0minFA 分别为 (1.4 5± 0 .32 ) %、(2 .4 2± 0 .5 6 ) % ,差异有统计学意义 (P <0 .0 5 ) 。 结论Objective: To investigate the uptake of isoflurane during semiclosed circuit anesthesia with low initial fresh gas flow (FGF) instead of conventional high initial FGF and to evaluate its feasibility for clinical use. Methods: Thirty ASA Ⅰ~Ⅱ patients undergoing upper abdominal were randomly allocated to one of two equal groups (n=15) : group A and group B. After tracheal intubation the patients were mechanically ventilated to maintain P\-ECO\-2at 32~36 mmHg. Both group FGF were O\-2 1 L·min -1 , during isoflurena wash in and isoflurena vaporizer in group A was set at 3% whereas 5% in group B. During maitenance of anesthesia isoflurena vaporizer and FGF were maintained unchanged. The target concentration (alveolar concentration, F\-A) of isoflurena was set at 1.2%. The mean time of isoflurena wash in to the target concentration, the inspired concentration (F\-I), F\-A of isoflurena and mean arterial blood pressure (MAP) at 60 min were recorded. Results: The mean time of isoflurena wash in to target concentration was (4±1) min in group A and (16±3) min in group B, MAP or F\-A at 60 min of isoflurena wash in in group A was (79±20) mmHg, (1.45±0.32) %, in group B was (75±17) mmHg, (2.42± 0.56) %. Conclusions: The semiclosed circuit anesthesia wash in with low initial fresh gas flow (FGF) and isoflurena is safe and feasible.
