DRFS在胸腰椎手术中的应用

摘　　要：目的 :评价撑开、复位、内固定系统 (DistractionReduceFixationSystemDRFS)在胸腰椎手术中的应用价值。方法 :对胸腰椎骨折 2 6例 ,结核 10例 ,腰椎滑脱 2 4例病人均行DRFS内固定及植骨融合术。结果 :随访 4～ 36个月 ,平均 14个月 ,2 6例胸腰椎骨折者生理曲度恢复 ,伤椎高度恢复 90 %,神经功能除 1例A级的无改善外 ,其余的均有 1～2个级别的提高 ;结核 10例 ,后突畸形矫正 ,脊柱稳定 ,植骨在 4～ 5个月内融合 ,无 1例复发 ;滑脱 2 4例 ,2 0例完全复位占 83.3%。无 1例出现断钉、折板、螺钉松动。结论 :DRFS材料优质、设计合理、操作简单 ,在术中同时完成撑开、复位、固定作用 ,效果优良 ,是胸腰椎手术中理想的内固定物。Objective:To evaluate the clinical result of DRFS for the surgery of thoracolumbar diseases.Materials:26 cases of thoracolumbar fractures,10 cases of spinal tuberculosis,24 cases of spondylolisthesis,were treated with DRFS internal fixatioin and bone grafting.Result:The mean period of follow-up was 14 months(from 4～36 months),26 cases of thoracolumbar fracture,the hights of the injured vertebrae were restored to 90 %,Frankel score of spinal cord function were improved 1～2 grades in all patients except 1 who had a score of A before operation.Deformity of kyphosis were rectified and spine were stabilized,bone graft were fused in 4～6 months.In 24 cases of spondylolisthesis,20 reached complete reduction(83.3 %).Nail breaking,plate breaking and screw loosen was not found.Conclusion:DRFS possessed a good quantity,were well designed,and easily operated in operation,meanwhile,it provided a good result of distraction,reduction and fixation.DRFS was an ideal internal fixation placement for thoracolumbar spine for its excellent result,good quantity,well design and simple operated in operation.
