摘  要:目的 评估红细胞平均体积 (MCV)差值法在鉴别血尿来源中的价值。方法 测定 12 9例血尿患者的血MCV和尿MCV ,计算两者之间的差值 ,用他来判别血尿来源 ,并与尿MCV法作比较。结果 以MCV差值≥ 9.70fl为诊断肾小球性血尿的标准 ,其敏感度为 0 .92 0 ,特异度为 0 .987,正确率为 0 .96 1;以尿MCV≤70 .75fl为诊断肾小球性血尿的标准 ,其敏感度为 0 .880 ,特异度为 0 .899,正确率为 0 .891。结论 与尿MCV法相比 ,MCV差值法具有较高的敏感度和特异度 ,可以作为筛检血尿来源的方法之一。Objective To evaluate the value of the difference of mean corpuscular volume (MCV) between urinary and blood erythrocytes (MCV-difference) in distinguishing the origin of hematuria. Methods The MCVs of urinary and blood erythrocytes (MCVU and MCVB) were measured in 50 patients with glomerular hematuria and 79 patients with nonglomerular hematuria, and the MCV-difference is defined as the difference of MCVB minus MCVU. MCV-difference method was used to distinguish the origin of hematuria and compared with MCVU method. Results When the cutoff of MCV-diffenence was defined as ≥9.70 fl for glomerular' hematuria the sensitivity was 0.920, the specificity 0.987 and the accurary 0.961; while the cutoff of MCVU as ≤70.75 fl was used, the sensitivity was 0.880, specificity 0.899 and accuracy 0.891. Conclusions MCV-difference method is more accurate than MCVU method, and may be used as a routine examination to distinguish the sourse between glomerular and non-glomerular haematuria.
