摘  要:本文通过对74例有冠心病体征、临床诊断排除其它心血管疾患且心瓣运动和形态正常患者的左心导管临床资料进行分析,描述了主动脉瓣开启造成左室压力波形出现大幅度波动的“过冲”现象,指出在左室形态、大小和舒缩运动定性正常时,此现象仍然发生。过冲现象造成主动脉瓣开启和,瞬时跨主动瓣压差可超过超声多普勒估测的在体正常值的最高限,并伴随左室压力在短时间内低于升主动脉压力,故认为瞬时最大跨主动脉瓣压差作为评价心瓣血流动力学特性的参数是无意义的。本文还给出人体左心循环的部分定量参数,分析了参数间相关关系,同时分析了左室形态、大小和收缩协同性对这些关系的影响,为人体循环模拟系统更好地实现对人体循环的模拟提供依据。In this observation, haemodynamic information from left heart catheteration on 74 patients with symptom of coronary heart diseases except any other heart diseases,especially, valvular heart diseases, was analysed. We discribed a kind of phenomenon named pressure excess, which is the vibration of Left Ventricular (LV) pressure accompanying with the opening of aortic valve. In the phenomenon, temporary trans aortic valve pressure gradient could be much higher than the maximum limitation of the normal value estimated by Doppler Echocardiography in vivo, and aortic pressure could be lower than LV pressure. The results show that the LV morphology, size and motion has no relation with the phenomenon. So maximum pressure gradient is of useless in appreciating the hydrodynamic character of heartvalve. Some quantitive parameters and straight line corelation between them were given in this article, and the influence of LV morphology, size and motion on the correlation were given also.
