Objective To investigate the possibility of using dipyridamole instead of acetazola mide as a stress agent in stress cerebral perfusion imaging. Methods Eight healthy volunteers were taken as controls, 23 patients suspected as suff ering from ischemic cerebral vascular diseases were taken as patient group. All subjects underwent rest, acetazolamide and dipyridamole stress cerebral perfusio n imaging separately within one week. Visual evaluation and region of interest ( ROI) method was us ed to compare cerebral perfusion under rest and stress conditions. Results The cerebral perfusion of the normal control group was symmetric und er rest and stress conditions. For the patient group, 8 patients were classified with normal cerebral blood flow (CBF). The CBF of the remaining patients were normal or ischemic under rest conditon. Under stress condition, the blood flow became e ven lower, or new ischemic areas occurred. The effects of dipyridamole and aceta zolamide were similar to each other in 5 patients. The effect of dipyridamole wa s stronger in 5 patients. The effect of acetazolamide was stronger in 4 patients . In general, the effects of the two drugs in showing the area with low flow res erve were much alike. Conclusion Dipyridamole can be used as a stress agent in cerebral perfusion imaging, taking the place of acetazolamide.
